- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E060104, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is a Total Small Business Set-Aside. All small business shall provide a breakdown of their costs to demonstrate that at least 51% of the cost of contract performance incurred for travel, supplies, training materials, etc. shall be performed and incurred by the small business (prime). All small businesses shall comply with FAR 52.29-14 Limitations on Subcontracting. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 001: Provide all travel, supplies, materials and personnel to provide occupational therapy services to the BIE, Tuba City Boarding School, Tuba City, AZ in accordance with the Statement of Work. The period of performance shall be from September 15, 2006 through June 30, 2007 with four option years. Each option year shall begin July 1 and end June 30. The award shall be made for a basic and four option years. Please provide your quote, with a breakdown of costs, for the basic year and for option years. STATEMENT OF WORK Scope of Work. The Contractor shall provide evaluations; screening for early intervention and as part of Child Find; provide direct therapy and consultative services; train school wide staff and parents. Contractor shall provide occupational therapy services. OCCUPATIONAL THERAPY: 1. The registered occupational therapist will complete evaluations and consult with paraprofessional and certified teachers concerning treatment programs. 2. Contractor shall develop a schedule for the evaluations and placement of students; the schedule shall be developed with input from the Special Education Department, Tuba City Boarding School. 3. The Contractor shall evaluate and prepare a summary report for each individual student's file. The summary report shall be completed and available for student Multi-Evaluation Team meeting as scheduled. 4. Recommendations shall be made to the special education teacher and the general education classroom teacher on a consultant basis. 5. The contractor shall provide school wide training and instruction to the staff of the school. Training will enable the staff members to become aware and deliver the appropriate direct services to students during the time between scheduled visits by the certified occupational therapist. 6. The contractor shall develop an IEP that will outline occupational therapy services delivered on both a consultant and direct therapy service basis. DIRECT OCCUPATIONAL THERAPY SERVICES: 1. Contractor shall provide direct therapy services individually or in small group setting, as appropriate. 2. Recommendations shall include adaptive equipment to enable optimum classroom performance. 3. Appropriate services according to the student's IEP therapy goals and objectives will guide and direct therapy services. 4. Contractor shall attend IEP, MDT/MET meetings pertaining to students receiving OT services. 5. Contractor shall prepare one IEP that will include other services that the student is receiving in other disability area(s). 6. Contractor will provide Extended School Year services for eight (8) weeks during summer break, if necessary, per IEP. 7. The contractor shall attend parent information meetings. 8. Contractor shall submit a monthly-summarized invoice. Invoice shall contain the following information: a. Purchase order number. b. Date services were rendered. c. Name of technician providing service. d. Description of services rendered. e. Time spent with student (hours, minutes, etc.). f. List student's initial on each invoice. g. Attach a student listing using full name (audit purposes) The certified service provider shall be aware that no discriminatory racially and culturally biased assessment be utilized for evaluation. Please include your TAX ID Number, CAGE Number (obtain at www.ccr.gov<http://www.ccr.gov/>), and DUNS Number when submitting your quote. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service in accordance with the four areas in the Breakdown of Costs Section ; and, provide a copy of their credentials, including but not limited the college diploma. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program in accordance with the Background Investigations Clause. The Offeror shall provide the lump sum cost with a breakdown of their cost to provide this service and respond to the Evaluation Factors delineated at the end of this synopsis.
