***This amendment is to extend the date offers are due to 4:00PM PST, 18 September 2006. The purpose of this extension is for offerors to review a revised Appendix A and revise their offer. Appendix A has been corrected to remove some facilities that do not need servicing in FY 07. A new copy of this Appendix A can be retrieved by emailing Lt Lora Speights at lora.speights@luke.af.mil*** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotes (RFQ). Submit written offers, oral offers will not be accepted, on RFQ F2U3146205A200. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12 and Defense Acquisition Circular 20060711. This procurement is being issued as 100% Small Business Set-Aside. The North American Industry Classification System (NAICS) Code is 562119 and the size standard of $11.5 Million applies to this procurement. The requirement is to pump and clean oil/water separators on Luke AFB from 01 October 2006 to 30 September 2007, in accordance with the attached Statement of Work (SOW). FOB Destination to Luke Air Force Base, Arizona 85309. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition. The provision at FAR 52.212-2, Evaluation--Commercial Items applies; inserted in paragraph (a) the following factors shall be used to evaluate offers: lowest price, technically acceptable. A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications--Commercial Items shall be included with each offer. The clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items applies to this acquisition. The clause at FAR 52.212-5(Dev), Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this solicitation. Specifically the following clauses and provisions cited are applicable to this solicitation: 52.214-21, Descriptive Literature.
