*** AMENDMENT TO SOLICITATION THIS AMMENDMENT IS TO REPLACE WAGE DETERMINATION 94-2525 REV 27 WITH 05-2525 REV 01 AND TO UPDATE THE STATEMENT OF WORK. PLEASE REVIEW THESE CHANGES. AMENDMENT TO SOLICITATION *** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F1B3R26247A100 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12 4 Aug 06. This acquisition is 100% set aside for small business concerns and is under North American Industry Classification Standards (NAICS) code 562991, with annual gross revenue not to exceed $6.5 million. Statement of Work: 1. GENERAL: The contractor will clean, treat and properly dispose of all oil, grease, sand and sludge in the grease traps and oil/water separators that are on Altus Air Force Base, OK. Collection stations are located adjacent to facilities 16, 39, 106, 185, 267, 301, 307, 317, 321, and 349. Contractor will provide all plant, supervision, materials, equipment, tools, vehicles, labor and incidentals to complete cleaning services to the satisfaction of the Contracting Officer. 1.1. Specific Tasks: Remove approximately 6,575 gallons of oil, grease, sludge, mud/sand and water from traps, pits and tanks at the facilities listed below: Commissary, Bldg. 16 Capacity: 990.65 Gal Officer Lounge, Bldg. #39 Capacity: 198.13 Gal Bowling Alley, Bldg. #106 Capacity: 550.00 Gal Flight Kitchen, Bldg. #185 Capacity: 392.30 Gal Fire Dept., Bldg. #267 Capacity: 602.07 Gal Base Chapel, Bldg. 301 Capacity: 648.38 Gal Club Altus, Bldg. 307 Capacity: 190.20 Gal Solar Inn, Bldg. 317 Capacity: 1,902.04 Gal Base Car Wash, Bldg. 321 Capacity: 300.00 Gal Vehicle Maint Car Wash, Bldg. 349 Capacity: 800.00 Gal 1.1.1. Thoroughly clean all traps and pits, adding back water in the trap as needed. Thoroughly wash trap, pit and tank walls, floors/bottoms and tops with pressurized hot water or steam, soap and detergents. Alternate cleaning methods will require submittal for technical review and subsequent approval by the Contracting Officer (CO) before they are implemented. 1.1.2. Deodorize and disinfect all traps, pits and tanks with a soap or a cleansing solution that is environmentally friendly and approved by the CO. 1.1.3. Ensure containment tanks or vessels are clearly labeled to identify the contents. 1.1.4. Inspect influent and effluent piping for obstructions or clogs. Report obstructions, immediately, to the QAE. 1.1.5. Dispose of all grease, oil, sludge, mud/sand and waste water, off the installation, at a certified disposal site. All disposals will comply with existing local, state, and federal regulations. Coordinate with the Altus AFB Environmental Section on handling and disposal of debris to off base disposal locations at (580) 481-7346 or 481-6198. The contractor is responsible for all permits or fees associated with the use of off-base disposal locations. 1.1.6. Site Clean-Up: Ensure each collection site is cleaned and surrounding areas are dressed up after servicing each trap, pit or tank on Altus Air Force Base. 1.1.7. Apparent Serviceable Material: Do not dispose of any material, which has obvious value and appears to have been inadvertently placed adjacent to the collection point, unless such material is designated by Quality Assurance Personnel (QAP) for removal. 1.2. Management, Staffing and Equipment: In addition to items specified above, the contractor shall furnish all equipment, parts, fuels, pumps, labor, cleaning material (liquids and non-liquids). No storage site is available on Altus AFB. 1.2.1. Personnel and Safety: Provide only qualified and knowledgeable personnel to perform the tasks required to properly empty and clean grease traps and pits as specified. Personnel will wear protective clothing, to include face shields, as necessary during cleaning operations and handling of waste materials. 1.2.2. Coordination of Work: Coordinate all cleaning operations with the QAP and the Contract Administrator (CA). Submit written work schedules showing areas to be worked, dates of work, and estimated time for completion of each cleaning service for approval and coordination fifteen (15) days in advance of each quarter?s work to be accomplished. Also, notify the QAP at (580) 481-7755 two (2) work days prior to arrival on the installation. Verification of field measurements and removal quantities is the responsibility of the contractor. 1.2.3. Vehicles and Equipment: The Contractor shall provide all vehicles required to fulfill the requirements of this contract. All vehicles shall be operated in accordance with base traffic regulations. Vehicles shall be in operable condition and meet local, state, and federal safety requirements. Vehicles shall be painted to present a neat appearance and have the Contractor?s name affixed on both sides of the vehicle. Vehicle repairs on base shall be limited to those minor in nature. 1.2.3.1. All grease collection vehicles shall be specifically designed for the purpose to pump out grease trap pit with all attachments needed to clean and deodorize the grease trap pits. 1.2.3.2. Contractor shall provide bulk containers. Container shall either be built in the vehicle or as an attachment. Containers shall have a minimum capacity of five hundred (500) gallons and present a neat appearance with proper caps/seals to prevent spillage while under transportation. 1.2.3.3. All towed cleaning equipment along with equipment transported over base roads shall be marked by a red/orange reflective triangle visible both day and night from 500 feet. Any load extending over the side of a vehicle shall be flagged. Any load extending 4 feet from the rear or 3 feet from the front of any vehicle or trailer shall be flagged. 1.2.3.4. Maintain all equipment, vehicles and holding tanks used in cleaning operations and for hauling waste products in a clean condition before entering the installation and during all extraction operations. 1.2.4. Operating Hours: Perform cleaning services Monday through Friday, from 7:00 AM through 5:00 PM. The contractor may request, in writing, to work during other or non-standard hours. Submit such a request, in writing, to the CO for review/approval. However, the contractor shall be prepared to adjust the work hours to meet changes due to mission restrictions or crisis situations, at no additional cost. 1.2.5. Recognized Holidays: The following days are recognized as holidays. Environmental and Hazardous Material/Waste (HM/W) Requirements: Prior to use on Altus AFB, the contractor shall provide a list of all hazardous materials to be brought on Altus AFB to the Base Hazardous Material Pharmacy, located in the Logistics Complex, Bldg 228. Contractor shall provide Material Safety Data Sheet(s) (MSDS) for hazardous chemicals or materials in use. 1.4.1. Prohibited Substances: No ozone depleting substances (refrigerants or any other compounds) will be released in any capacity during performance on this contract. No paint with a lead content of 0.06% or greater will be used in any capacity on this project. No asbestos containing materials (ACMs) are allowed on this contract. 1.4.2. HM/W Encountered by the Contractor: Immediately notify the CO upon encountering any material not identified in the contract documents thought to be hazardous that could jeopardize the safety of workers or personnel in the area. The Government is responsible for characterization, transportation, storage and disposal of the waste if necessary. 2. SERVICE DELIVERY SUMMARY: The contractor service requirements are summarized into performance objectives that relate directly to essential services. The stated performance thresholds are the minimum acceptable levels of service for each stated requirement. All aspects of the contractor performance are subject to government surveillance, and absence of any contract requirement from the SDS shall not detract from its enforceability nor limit the rights or remedies of the government under contract clauses. 2.2. Quality Assurance: A government representative from 97 CES/CEOE will assess the contractor?s performance and adherence to standards. The QAP will record results of inspections, noting the facility number, date and time of inspection. If inspection reveals unacceptable performance, the QAP shall notify the Contract Administrator (CA). The CA will then notify the contract manager and require reperformance of the service (referred to as ?call back?). The contractor shall be allotted time as agreed to by all parties, but no longer than 24 hours, to return to Altus AFB and accomplish corrective action/work. 2.1. GOVERNMENT FURNISHED PROPERTY and SERVICES: 3.1. Government Furnished Property: The government will not provide any facilities, equipment, supplies or materials. The government will furnish without cost to the contractor a reasonable amount of electricity and potable water in connection with the contract performance. 3.2. Call 911 for fire reporting or medical emergencies. (End SOW). A site visit has been scheduled for 10:00 AM CDT 19SEP06. Contractors interested in attending the site visit will need to register with one of the points of contact listed in this solicitation before 4:00 PM, 18SEP06. The meeting location for the site visit will be Altus AFB south gate (Located on HWY 62). All work shall be performed IAW SOW.
