Description
The purpose of this amendment is to extend the solicitation response date and change the following in the Performance Work Statement, Section I. paragraphs 1 and 3: From: Vendor will lease to David Grant Medical Center (DGMC) 101 Bodin Circle Bldg 777, Travis AFB, California a mobile PET/CT scanner, including all the transportation, tractor, trailer, computer hardware and software and any other equipment necessary to provide stated services and in which the scanner will be transported and housed also referred to the ?equipment?. The vendor will provide all equipment and software required to perform PET/CT exams to include the mobile PET CT imaging unit with capability to save images to a disk and transmit images to DGMC electronically via a CAT 5 line for use by DGMC providers for interpretation of images. The PET/CT equipment is, and shall at all times be and remain, the personal property of the contractor. The vendor will make the equipment available to DGMC?s patients at 101 Bodin Circle Building 777, Travis Air Force Base, California 94535 one Monday per month for 10 hours, beginning on or about 15 Sep 2006 for a full 12 month period. The contractor shall comply with the Joint Commission Accreditation Healthcare Organization (JCAHO), and all Federal, State, and local laws applicable to the performance of work. Vendor will also provide a technologist to operate the mobile PET/CT scanner. Vendor will provide all training required which will consist of at least one DGMC technologist working side by side with a qualified vendor technologist full time to allow DGMC the option to fully staff the unit at a later date. If DGMC staff?s the unit, cost of the services will be adjusted as appropriate with the mutual consent of the government contracting officer and the vendor. The vendor?s technologists will be licensed in California and the vendor shall provide proof upon receipt of written request from DGMC.
