Description: This is a combined synopsis/solicitation for research and development as supplemented with additional information included in this notice. Proposals are being requested and solicitation W81XWH-06-R-0027 will be issued. The objectives of this integra tion project are two-fold. Objective 1 is to conduct a proof-of-concept study that will investigate the feasibility of using Comprehensive Diabetes Management Programs (CDMP)AHLTA- compatible, web-based, stand-alone modules and algorithms for diabetes c are within AHLTA. Objective 2 is to assess the reliability of the Data Assessment Tool Kit (DATK) to provide accurate information when accessed on-line versus a computer in the clinic.
