Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. The solicitation number for this procurement is FA4803-06-QA371 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-12, Defense Federal Acquisition Regulation Supplement Change Notice (DCN) 20060808 and Air Force Acquisition Circular (AFAC) 2006-0515. For informational purposes the North American Industry Classification System code is 423910, Sporting and Recreational Goods and Supplies Merchant Wholesalers. This acquisition is set-a-side 100% for Small Businesses. This is a firm-fixed price procurement using commercial item procedures and award selection will be based on best value. Furnish all labor, tools, equipment and material, to recondition 12 playgrounds in accordance with the statement of work. The contractor that is selected must meet all state and federal regulations and be state certified in all safety aspects of playground installation. The following FAR provisions also apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items. The offeror's quote shall contain the following information: offeror's quote number, date, name, address, terms of any warranties, price, any discount terms, best delivery date for FOB destination, and acknowledgement of all RFQ amendments (if applicable). Quote shall also contain all other documentation specified herein. Late offers: Quotes or modification of quotes received at the address specified for the receipt of offers after the exact time specified for receipt of offers will not be considered. Offerors shall include completed copy of the Online Offeror Representations and Certifications Application. Response Time: All quotes will be accepted at the 20th Contracting Squadron (LGCP) located at 305 Blue Jay Street, Shaw AFB, SC 29152, no later than 1600 EST on 25 September 2006. Facsimile and electronic mail proposals will be accepted. STATEMENT OF WORK: Military Family Housing Playgrounds 1. Contractor shall furnish all labor, tools, equipment and material, to recondition twelve (12) playgrounds in the following Military Family Housing areas (Housing Office, Buckeye Circle, Buckeye-Sycamore, Hickory, 5000 Shoppette, Poplar Circle #1, Poplar Circle #2, Banyon-Beech Circle, Cherry St., Plum Street, Rhodes Ave., Chestnut.) The contractor shall remove and replace engineering wood fiber in the swing and playground areas. Contractor shall till areas that contain loose fill sand material. Contractor shall repair replace broken or damaged plastic borders. 2. Contractor shall remove all unsafe playground equipment and eliminate all safety hazards. Contractor will not be installing new equipment. 3. Roll off dumpsters will be furnished for the contractor to dispose of debris. Contractor shall dispose of all debris at the end of each work day into the provided roll off dumpsters. 4. Contractor can begin work within 30 days after contract is granted. 5. The inspection of the completed work will be made by the 20 CES Housing Maintenance Office and Quality Control Inspector. 6. The contractor must notify the 20 CES Housing Maintenance Office (Bruce T. Mack 895-2439) and Quality Control Inspector (TSgt Lopez 895-2219) at least five working days prior to start of work and upon completion. 7. The selected licensed contractor must meet all state and federal regulations and be state certified in all safety aspects of commercial playground installation. HOUSING OFFICE PLAYGROUND- Remove 2 spring items (2), Remove wooden seesaw (1), Rubber material around 4-seat spring item (approx. 200 sf) (1), Till area that contains loose fill sand material (1), Reverse swing hanger bolts to eliminate safety hazard (8). BUCKEYE CIRCLE PLAYGROUND- Engineered wood fiber (swing area and top off play area) (24), Till area that contains loose fill sand material (1), Reverse swing hanger bolts to eliminate safety hazard (4), Borders for swing (22), Install bolts on bubble panel (1), Replace S-hooks and bolts (1), Installation of plastic borders (1), Installation of EWF (1). PLAYGROUND 11 BUCKEYE/SYCAMORE- Add 48 cy of EWF (48), Installation EWF (1). SYCAMORE/HICKORY PLAYGROUND- Reverse swing hanger bolts to eliminate safety hazard (4), Till area that contains loose fill sand material (1), Replace S-hooks and bolts (1), PLAYGROUND 14 SHOPPETTE- Till the non tiled use zone including the swing area (1), Note: For the non-rubber area remove the existing safety surface material and add engineered wood fiber Replace 4 Bubble Panels (4), Repair tiles (1), Repair steering wheel (1), Installation of above items (1), Note: Loose tiles will be removed and new adhesive applied to those tiles. POPLAR CIRCLE 1 PLAYGROUND- Engineered wood fiber (59), Installation of EWF (1), Till loose fill area around play structure (1). POPLAR CIRCLE 2 PLAYGROUND- Engineered wood fiber (36), Installation of EWF (1). BANYAN & BEECH CIRCLE PLAYGROUND- Remove spring items (4), Remove swing (1), Remove merry-go-round (1), Remove metal climber (1), Remove slides (2), Till loose fill area around play structure (1). CHERRY STREET PLAYGROUND- Remove spring items (5), Remove swing (3), Remove merry-go-round (1), Remove metal climber (1), Remove slides (1), Remove seesaws (2). PLUM STREET PLAYGROUND- Reglue loose rubber tiles (approximately 90) (1), Till loose fill area around play structure (1), Engineered wood fiber (14), Installation of EWF (1). PLAYGROUND 5 RHODES AVE- Add 140 cy of EWF (140), Replace 4 Plastic Borders (2-6' and 2-2' borders) (4), Installation (1). CHESTNUT PLAYGROUND- Engineered wood fiber (56), Installation of EWF (1), Rotate swing hanger bolts (8), Plastic borders (40), Installation of borders (1).
