(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Request for Proposal (RFQ) DHAM50404. (iii) The RFQ document and incorporated provisions and clauses are those in effect through FAC 2005-05. (iv) The resultant contract will be 100% set aside for small business. The NAICS code is 541519, other Computer Related Services. The size standard is $23M average annual receipts over past three years. (v) Provide a specific make and model pricing for NEW items for the following CLINs. (vi) CLIN 0001: Provide Polycom VSX 8400 Video Teleconferencing System as described in the Equipment List; The Contractor shall provide all equipment, qualified personnel, labor, supervision, management, and materials to provide Video Teleconferencing System, installation, assemble, test, and train so as to achieve a professional appearance and complete performance. Once complete, the equipment is to be operational and capable of performance. CLIN 0002: Basic Maintenance Technical Support (BMTS) as described in the Equipment List, and CLIN 0003: Training Support as described in the Equipment List. (vii) Delivery address is 1700 Defense Pentagon 3DI067, Washington, DC 20301-1700. FOB destination; acceptance at destination. Earliest Delivery is requested. Award will be based on technically acceptable, lowest price. (viii) FAR 52.212-1 applies to this acquisition.
