Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement, F3G3MH6165A400 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12. The NAICS code for this solicitation is 611620. The size standard for a small business under this NAICS code is one that is equal to or less than $6,500,000 of Annual Total Income. This Request for Quotation is set-aside for small businesses. This is a commercial purchase. Base and Option CLINs 0001/1001/2001/3001/4001 ? 540 session/class ? Aerobic Classes. 1. Contractor shall furnish instruction for aerobics/group exercise training by teaching classes in various types of group exercise activities. Classes will be taught at the Nellis AFB Sports and Fitness Center to authorized patrons that may include both men and women, active duty, retired, civilians and reservists. 2. The Sports and Fitness Center will schedule classes. 3. Contractor will certify that all instructors have a current CPR certification and current instructor?s certification from ACSM, ACE, IDEA, REEBOK, YMCA, AFAA, SCHWINN or other nationally recognized certification organization. 4. Contractor will insure that: a. Instructors arrive 15 minutes prior to the scheduled class start time. b. Instructors set up and take down all stereo equipment. c. Instructors are safety conscious and begin EACH class with a safety briefing. d. Instructors will provide a safe, effective group exercise class that is positive and motivating to the students. e. Instructors dress appropriately to teach a group exercise class. f. Instructors sign in for each class taught. Failure to sign in may be reason to withhold payment for that class. 5. The Athletic Director may reject any instructor. 6. Contractor will report any equipment malfunction, injuries, or problem with the class to the Athletic Director or his representative within 24 hours. 7. Contractor will meet with the Sports and Fitness Center staff monthly to receive a class/instructor schedule. 8. Contractor will provide all music for the class. Bikes and other equipment needed for classes will be provided by the Sports and Fitness Center. Classes need a minimum of 5 participants to be held. 9. The Athletic Director will be the final approval authority for the type of class offered. 10. Class format will be decided by the Athletic Director, after consultation with Contractor. 11. Marketing of the program will be done through the 99 SVS Marketing Office and the base newspaper. Contractor may submit for approval articles, flyers, etc. for publicity purposes. All articles will be submitted through the Athletic Director for approval. 12. Classes may be any kind of group exercise classes, to include but not limited to aerobics of all designs, cycling, yoga, jazzercise, circuit training, and treading. The type and nature of the class and program will be determined by the Athletic Director. All scheduling will be done by the Sports and Fitness Center staff. Quotes should be in the following format; price for each line item independently and price for combined line items. Award Basis: Best Value. The following factors will be used to evaluate the offer: Technical Capability (copies of certificates), Past Performance and Price. Technical capability and past performance are less inmportant than price.
