- The United States Army Yuma Proving Ground (USAYPG), Arizona, has a requirement for service and maintenance of metal shop machinery located at building 3504(W) and building 3490 (S). This a combined synopsis/solicitation for commercial services prepa red in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12 (04Aug06) and Class Deviation 2005-o0001 and Defense Federal Acquisition Regulation Supplement (DFARs) current to DCN20060908 edition. This acquisition is being solicited as a 100% Small Business Set-Aside. The North American Industrial Classification Code (NAICS) appl icable to this acquisition is 811310 with a size standard of $6.5 Mil. The contract will include a 12-month base period and (4) one-year option periods. The contract shall consist of ten Contract Line Item Numbers (CLINS) as follows: CLIN 0001 Furnis h all labor, material and equipment necessary to provide an annual maintenance of all equipment Base Period 01 Oct 2006 30 Sep 2007; Unit of Issue Month; Quantity 12. CLIN 0002 Perform minor and emergency repairs. Include hourly labor rate (re gular and after hours), initiated by a service call. Response time for calls shall be within 24-48 hours. Base Period 01 Oct 2006 30 Sep 2007; Unit of Issue Lump Sum; Quantity 1. CLIN 0003 Furnish all labor, material and equipment necessary to p rovide an annual maintenance of all equipment Option One 01 Oct 2007 30 Sep 2008; Unit of Issue Month; Quantity 12. Offerors shall bid a fixed monthly price for the base period and each option period. CLIN 0010 Perform minor and emergency repairs. Include hourly labor rate (regular and after hours), initiated by a service call. Response time for calls shall be within 24-48 hours. Option Four - 01 Oct 2010 30 Sep 2011; Unit of Issue Lump Sum; Quantity 1. For CLINS 0002, 0004, 0006, 0008 and 0010 the government will establish a Not to Exceed (NTE) amount for emergency repair services. The Government intends to award a contract to the responsible offeror whose offer conforming to the proposal will be the most advantageous to the Government based on past performance and price. Offerors shall provide company name, address, phone/fax number, and point of contact information on (5) recent jobs that demonstrate experience in maintaining and repairing same or similar equipment as listed on http://www.yuma.army.mil/contracting/index.html Offers that fail to furnish required representations or technical information as required by FAR Clause 52.212-1 and 52.212-3 cited below or reject the terms and conditions of this solicitation may be excluded from consideration. It is anticipated payment will be made by Government VISA Payment Card.
