- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation W912J2-06-2003 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. If requesting by FAX, you may send your request to 414-944-8781, ATTN: James Ensign. The North American Industry Clas sification System (NAICS) number is 334210 and the business size standard is 1000. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. The proposed contract listed here is 100% Total Small Disadvantage Business Set Aside. The requirement is for an emergency crash system that meets the following requirements: 1. Designed to work with or without a Private Branch Exchange (PBX), or both. 2. Capacity of 40 circuits that can ring down (dedicated-No PBX needed) or Public Switched Telephone Network (PBX needed). 3. Ability to program any phone to initiate a call on emergency crash system is crucial. 4. Contacted personnel should be able to go off hook. 5. If calling in the system from the PBX, personnel should have the option to di al a code before initiation begins. 6. Cannot be limited by a fixed number of ports. 7. Ability to accommodate the base 911 service. 8. Programmable in 8 port increments. 9. Should utilize our existing PBX lines. 10. An emergency call should be able to cal l into the phone system and the system should be initiated among strategic locations to ring simultaneously. Once picked up they are on conference immediately. 11. Designed to be initiated at Base Operations by going off hook in an emergency. 12. One phone call should trigger multiple phones to ring simultaneously in strategic locations. 13. Once answered everyone should be on a conference call and be able to talk simultaneously during the call or be set to listen only mode. 14. Time between initiation and call ring down should be within one second. 15. Specific base personnel should be able to view the activity through a live computer graphical user interface 16. Must meet the requirements of Air Force Instruction 13-213 (Available at http://www.e-publishin g.af.mil/pubfiles/af/13/afi13-213/afi13-213.pdf Instruction requirement is as follows: 3.2.2.3. Secondary Crash Net (SCN): AM Ops shall have SCN activation capability with an additional extension for monitoring and training purposes. 3.2.2.3.1. All stations on the SCN will be on lines dedicated to the dissemination of emergency information that affects airfield or aircraft operations. The SCN may be a separate telephone instrument or integrated into a multi-line call center. The teleph one system must be equipped with a visual feature that activates as each two-way party on SCN picks up the handset. All stations on the SCN must used a noise reduction feature such as push-to-talk handsets or Confidencor (Trademark of National Communicatio ns Inc.), that filters out background noise. IAW 13-214 3.2.2.3.5. Only use the SCN to relay information critical to aircraft and airfield operations (e.g., hazardous weather warnings, in-flight emergencies (IFEs), ground emergencies (GEs), Force Protectio n Condition (FPCON) levels, DCG activations/recalls, bomb threats or terrorist activities). Use other forms of communication to relay non-critical information. The system will be installed in our communications room will be responsible for maintenance onc e complete. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional provisions and clauses appl y: 52.212-2, Evaluation-Commercial Item, the Government will award a contract resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors consi dered. The following factors shall be used to evaluate offers: 1) Technically acceptable; 2) Past Performance; and 3) Price. Technically acceptable is more important than past performance and price. Technically acceptable will be evaluated based on the fun ction and operation of the item. To be technically acceptable the item shall meet the Government's specifications. Offerors shall submit descriptive literature, product samples, technical features, and warranty provisions. Past performance is considered m ore important than price. Past performance will be evaluated to ensure satisfactory business practices and timely performance. Price will be considered as the least important factor. Price will be evaluated to determine its fairness, completeness, and reas onableness as it relates to the items offered. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer. Quotes received without the completed copy of the provision at FAR 5 2.212-3 or statement of availability on ORCA will be considered non-compliant. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to includ e clauses: 5, 9, 14,15,16,17,18,19, 20, 21, 23, 24, 26, 31, and 32. The following additional clauses apply: CCR Clause: Attention: You must be registered in CCR to be considered for award. FAR 52.204-7, Central Contractor Registration (Oct 2003) (B)(2) the offeror shall provide its DUNS or, if applicable, its DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offeror is registered in the CCR database.
