Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation issued. STATEMENT OF WORK: Quotes are being requested and a written solicitation will not be issued. The solicitation number is 247- 0078-07 and is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-12, NAICS is 325412. The Department of Veterans affairs Birmingham, AL requires the following; (1) Contractor shall provide all labor, supervision, and transportation necessary to provide daily deliveries to the Department of Veterans Affairs Medical Center, (VAMC) Nuclear Medicine (Hot Lab) Room 2E-112, located 700 South 19th Street Birmingham Alabama 35233. The VAMC - Nuclear Medicine Service personnel or physician(s) via fax and/or phone shall place an order/s by 12Noon (cut-off time) one day prior to the scheduled appointment. (2) All radiopharmaceuticals/orders must be within +1- 10 percent (%) of the doctor?s prescribed order allowing only for correcting calibration data and time. All radiopharmaceuticals/orders must be in total compliance with the Nuclear Regulatory Commission, State of Alabama and/or other Federal regulations regarding radioactive materials/shipments as required by law. (3) Doses are to be delivered in syringes, 7-10 millicuries per dose; this will include a one (1) millicurie vial for use in calibration. Doses will be brought in Syringe(s) of F-18 Fludeoxyglucose, calibrated for 2:00pm, CST, as requested and/or as ordered by the Nuclear Medicine Physician (s)/Technologists. For each FDG dose ordered a quality control dose will be included at no additional charge. All work associated will be in accordance with the requirements of the Nuclear Regulatory Commission (NRC). Contractor shall provide copy of the NRC Licensure with their bid. (4) Deliveries will be made by 8:00am CST, excluding federal holidays; and will be precalibrated for use as ordered/requested by the Nuclear Medicine Staff. All shipping containers must be free of radioactive contamination both inside and outside container(s). All containers shall be in compliance with state, federal agencies (i.e., Department of Transportation (DOT), and NRC regulations. There will be no delivery charges and deliveries will be made as requested and/or as necessary to provide patient care. (5) In the event of mechanical failure/s contractor will still be held responsible for taking action as necessary to provide delivery of FDG with no additional charges or cost to the VA Medical Center. (6) The government reserves the right to add, reduce and/or cancel, at no charge any order at any time prior to the scheduled appointment by giving the contractor one hour?s notice prior to delivery. Contractor shall agree to a no charge for dose/vials not used due to unforeseen circumstances (i.e., patient hardship, veterans? death, inclement weather, etc.) (7) Medical Center Working Hours: Monday through Friday 7:00am to 4:30pm excluding federal holidays and any other day specifically declared by the President of the United States to be a National Holiday. (8) Invoices will be paid monthly in arrears upon receipt of statement/properly submitted invoice which must include the purchase order number. The SCHEDULE OF PRICES FOR THE PERIOD BEGINNING OCTOBER 1 2006 THROUGH SEPTEMBER 30,2007 SHALL BE LISTED AND PRICED AS FOLLOWS: Estimated Doses 200 ea. Calibrated FDG Doses? $_______ ea. $_________ total. Submit offers on Company letterhead stationary in accordance with FAR 52.212-1 Instructions to Offerors commercial items. Acceptance time will be 60 days from offer due date. The following provision apply to this solicitation: 52.212- 1,52.212-3 Representations and Certifications-Commercial Items; 52.212-4 Contract Terms and Conditions; 52.204-7 Central Contractor Registration; 52.212-1 Solicitation Provisions Incorporated by Reference; 852.270-4 Commercial Advertising; 852.271-70 Nondiscrimination in Services Provided Beneficiaries; 52.252-2 Clauses incorporated by reference 52.232-18 Availability of Funds; 52.232-38 Submission of Electronic Funds Transfer Information with offer; 52.233-2 service of Protest; 52.233-4 Applicable Law for Breach of Contract Claim. Interested parties are required to submit prices, clear and convincing data substantiating their capabilities and qualifications to satisfy this requirement. The Government contemplates award of a firm fixed price single award contract resulting from this solicitation. All offerors responding to this announcement shall submit an offer along with the information required in 52.212-1 and a completed copy of provision 52.212-3, to Contracting Officer, Central Alabama Veterans Health Care System, East Campus 2400 Hospital Road, Tuskegee, AL 36083 by 2:00PM local Time, 09/28/2006.
