- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation FA4877-07-Q-0073 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-12. This procurement is being issued as a 100% SMALL BUSINESS set-aside under NAICS code 711130 and small business size standard of 500 employees. Description: Protestant Keyboarist. Statement of Work For Protestant Traditional Organist/Pianist/Choir Accompanist FY 07 I. General Description of Services: The contractor will provide musical accompaniment for congregational singing at the 0830 Protestant Worship Service held every Sunday at the Hope Chapel. Period of performance is 1 October 2006 through 30 September 2007. II. Specific Tasks: The contractor shall be responsible for the following: a. The custodial responsibilities for musical supplies such as songbooks, sheet music, etc. b. Building security (insuring that all doors and windows are locked before departing facility) and conservation of energy (turning off all lights, equipment, etc.). c. Insuring all musical activities are coordinated with the chapel staff, when necessary utilizing proper chapel forms, and verifying that proposed activities are included in the chapel calendar. d. Musical worship for the entire service--from prelude to postlude for each service. e. Music accompaniment and rehearsal, contractor shall be in place and ready to perform 30 minutes prior to scheduled event. f. Total maximum sessions to be provided are 52 Sunday performances and 26 specials (Holy Days, rehearsals, special events/activities). III. Qualifications: The contractor must have a minimum of two years experience playing music. Contractor shall be able to read and transpose music. Contractor shall be albe to play piano and organ (ability to play synthesizer is desired, but not required). IV. Musical Selections: Music selections will be specified by or acceptable to the chaplain in charge of the service. Those chosen by the contractor will be appropriate church music for the type of worship service being conducted and related to the church season. The contractor must be able to play hymns out of the Chapel hymnal to include the prelude, postlude. V. Rehearsals: The contractor will meet, as a minimum, once monthly with the Traditional Worship Chaplain for purposes of coordinating the musical program. Weekly rehearsals must be at least 60 minutes in duration. The praise team warm-up time prior to a service will be considered part of the service. VI. Substitutions: If a substitute is required, he/she will be of at least equal competence of the contractor and acceptable to the Protestant Chaplain assigned to the Traditional Worship Service. Notice of the substitution will be given to the Wing Chaplain or his/her designee 24-hours prior to the scheduled service or function, when possible. VII. Certification of Services: The contractor will provide the Chaplain or representative with a monthly certification of services rendered on forms provided by the Wing Chaplain or representative. VIII. Government Furnished Property: The government will provide the following at no cost to the contractor: a. Music required for the programs b. Required instruments (except a guitar) IX. Desired Experience: The contractor shall submit a resume for this requirement. Desired experience is to have Organist/Pianist/Choir Accompanist experience in church or chapel setting, ability to read and transpose music, have knowledge and understanding of the various forms of music used in different Protestant worship settings, ability to work with individuals from different denominational backgrounds, experience in application of music in church setting, current technical skills are required. Before a contract is awarded, the Senior Protestant chaplain and/or pastor of the Traditional Protestant Service must conduct an interview and audition to ensure the contractor meets the experience stated above. X. Security Requirements: a. Security Training. All contractor employees shall receive initial and recurring security education training from the sponsoring agency?s security manager. Training must be conducted in accordance with DoD 5200.1-R, Information Security Program, and AFI 31-401, Information Security Program Management. Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, The Air Force Installation Security Program. b. Pass And Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-government owned vehicles. c. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc., and turn them in to the issuing office. d. Traffic Laws. Contractor employees shall comply with all base traffic regulations. Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones is strictly prohibited while driving on Davis-Monthan AFB. e. Random Personnel and Vehicle Searches. Contractor employees are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action. f. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB. g. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material. FOUO or FOIA material discovered by the contractor or contractor employees shall not be retained by the contractor and will be surrendered immediately to an appropriate Government authority. h. Reporting Requirements. Contractor employees shall immediately report to an appropriate government authority any information or circumstances of which they are aware that may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information. i. Physical Security. Contractor employees shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured. j. Key Control. The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government. (1) The contractor shall immediately report any occurrences of lost or duplicated keys to Quality Assurance Personnel. (2) In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the contractor. (3) The contractor shall prohibit the use of keys and/or cipher lock combinations, issued by the government, by any persons other than the contractor?s employees and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas. XI. Criminal History Background Check: According to DODI 1402.5, P. 7-1, Mandatory Disqualifying Criteria includes, ?Any conviction of sexual offense, a drug felony, a violent crime, or a criminal offense involving a child or children.? A list of discretionary criteria is provided in the DOD instruction. If you have discretionary criteria, your chaplains will assist you in an appropriately confidential evaluation.
