- THE PURPOSE OF THIS MODIFICATION IS TO CANCEL ORDER IN ITS ENTIRETY. THANK YOU This is a combined/synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, FA4427-06-Q-0008, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12 and Defense DCN 20060814. The associated Standard Classification (SIC) is 7629. The North American Industry Classification System is 339112 and the number of employees is 500. The requirement is unrestricted.. The government intends to solicit and award this purchase order for the following item: CLIN 0001: Contractor shall furnish materials, tools, parts, labor, transportation, and other necessary item to provide an on-site surgical instrument sharpening and repair services, to include, but not limited to sharpening, repair, instrument color coding, needleholder insert, replacement, laparascopic instrument building, and complete electrosugical reinsulation as defined in the Scope of Work. On-site service location is David Grant Medical Center, Bldg 777 Travis AFB, CA 94535. CONTRACTORS ARE ASKED TO SUBMIT COST PER INSTRUMENT AND NOT PER HOUR OR LABOR RATE. Equipment List will be provided by fax upon request. FOB is Travis Air Force Base. The following provisions or clauses apply to this acquisition. Offerors must comply with all instruction contained in FAR 52.212-1, Instruction to Offers-Commercial. FAR 52.212-2-Evaluation-Items paragraph (a) to this provision is completed as follows-Evaluation Factors: PRICE ONLY. FAR 52.212-3-Offeror Representation and Certification-Commercial Items, a completed copy of this provision shall be submitted with the offer. FAR 52.212-4-Contract Terms and Conditions-Commercial Items will be incorporated into any resultant Purchase Order by reference.
