- Provide a comprehensive Wellness Program for employees and families of the Stanislaus National Forest which includes, but is not limited to, the following assessments and program components: Blood Chemistry and Coronary Risk Profile, Physical Fitness Testing, Lifestyle Health Risk Profile, Stress Profile, Computerized Individual Summary, Computerized Worksite Summary, Computerized Forest Summary, Data Interpretation, Provide consultation to Contracting Officer, Provide consultation to Forest Wellness Committee. The work sites listed below under ?Stanislaus National Forest? are referred to as the Districts/Units and shall be treated separately in all reports. Offices and Estimated Number of Employees (totaling 185) include: Supervisor?s Office at Sonora 75, Mi-Wok RD at Mi-Wuk Village 30, Calaveras RD at Hathaway Pines 30, Summit RD at Pinecrest 20, Groveland RD at Groveland 30. Note: The figures are estimates only. Additional employees may decide to participate in the Wellness Program after the initial start-up phase. The Contractor shall address its ability to include these employees in the Program.
