Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Request for Quote (RFQ) 580-25-06 is assigned to this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12. NAICS 339112 with a size standard of 500 employees applies. This will be a sole-source procurement for the Operative Care Line at the Michael E. DeBakey VA Medical Center, Houston, TX. Only one vendor, Intutive Surgical, is known to offer this equipment which can perform minimally invasive, laparoscopic surgery and has FDA approval, thus the system will be procured through full and open competition. Blade Protector 1 ea; Wide Angle Camera Starter Kit: 16. Wide Angle Camera 1 ea; 17. Scope, 0 Degree 1 ea; 18. Scope, 30 Degree 1 ea; 19. Universal Scope Alignment Target 1 ea; 20. NOTE: Quote price FOB Destination with delivery to be made to the VA Medical Center in Houston, TX 77030-4298 within 30 days after receipt of order.
