DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation document. Request for Quotes (RFQ) for Vocera communication system for the James A. Haley Veterans? Hospital, Tampa Florida. This is a brand name only (Vocera) solicitation. A written solicitation will not be issued. This notice incorporates provisions and clauses in effect through FAC 2005-12. This solicitation is full and open. The associated NAICS is 334290. A Firm-Fixed Price contract is anticipated. System Description: Vocera with enterprise licensing. Professional Services: Vocera Professional Services ? qty. 1, Project Management ? qty. 1. The following provisions and clauses apply to this RFQ: 52.212-2, Evaluation - Commercial Items. Evaluation criteria: The RFQ includes the Commercial Items evaluation provision (52.212-2) which provides that award shall be made to the responsible contractor whose quote is most advantageous to the Government, price and other factors considered. The significant evaluation factors are i) price, ii) past performance, and iii) adherence to system requirements. The evaluation factors are equal in importance. The resulting firm fixed price contract shall also incorporate the requirements of the clause FAR 52.212-4, Contract Terms and Conditions-Commercial Items applies, FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items applies with the following applicable clauses in paragraph (b): 52.203-6 Alternate I, 52.219-4, 52.219-8, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-39, 52.225-1, 52.225-13. The following additional FAR and VAAR clauses apply: 852.237-70 Contractor Responsibilities, 852.210-71 Guarantee, 852.270-4 Commercial Advertising, 852.233-70 Protest Content, 852.233-71 Alternate Protest Procedure. PRODUCT MODIFICATION, REMOVAL OR RECALL: If any product receiving an award on this solicitation requires modification, is removed or recalled by the contractor or manufacturer, or if any required modification, removal or recall is suggested or mandated by a regulatory or official agency, the following steps will immediately be taken by the contractor: (a) Provide the official notification to all agencies and DVA Hospitals who purchased the product. (b) Notify the Contracting Officer in writing, by the most expeditious manner possible. The notice shall be two copies of the official notification and shall include, but not be limited to the following: (1) complete item description and/or identification, order numbers from customers, and the contract number assigned as a result of an award on this solicitation; (2) reasons for modifications removal or recall; and (3) necessary instruction for return for credit, replacement or corrective action. Parties interested in responding to RFQ may submit their quote in accordance with standard commercial practice (i.e. quote form, letterhead, etc.) and must include the following information: RFQ number, offeror's complete mailing and remittance addresses, discount terms; unit prices and extended total price for all items, DUNS number, and completed FAR 52.212-3, and three references for evaluation of past performance of same or similar systems. All contractors are required to register in the Central Contractor Registration (CCR) database before contract award. Contractors are responsible for the accuracy of information added to CCR. To register, go to <http://www.ccr.gov/>. Quotes may be submitted by mail to the attention of David Chittenden, Procurement Business Division, James A. Haley Veterans? Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612. Or quotes may be submitted via fax (813) 903-4838, attention David Chittenden. The VA intends to evaluate offers and make contract award without discussion.
