THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6 AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. ***************************************************************** Purchase Request Number: F3N7156170A005 - Request for Quotation ? This document incorporates provisions and clauses in effect through the Federal Acquisition Circular 2005-07. ***************************************************************** Line Item 0001: Services, Non-Personal: Contractor to provide all preventive maintenance, calibration, labor for all on-call repairs and preventive maintenance, all travel costs, parts, and hotline service. All supplies are included in contract. *** Period of Performance: 01 Oct 06 ? 30 Sep 07 Quote should also include one option year from 01 Oct 07 ? 30 Sep 08. *** STATEMENT OF WORK 1. Contractor shall provide all labor, tools, materials, equipment, facilities, transportation and all other items as necessary to provide laboratory services as specified in this statement of work. 2. Specimens pick up and delivery. 2.1. Contractor shall pick up laboratory specimens in Building 208, 470th Air Base U.S. Clinic Laboratory, Geilenkirchen, Germany. 2.2. Specimens will be picked up daily between the hours of 0900-1400hrs, Monday thru Friday excluding German Holidays. 2.3. Specimens will be shipped to contractor laboratory to arrive within 24 hours of pick up. Transportation conditions must ensure no specimen deterioration occurs. 3. Result reporting. 3.1. Contractor will provide results via Internet and will fax any critical results to 2451-65359. 3.2. Reports will meet all College of American Pathologists (CAP) requirements and suitable for reproduction in a Xerox type machine. 3.3. The contractor must render test results within seven workdays of receipt of the specimen. 4. Quality of work performed. 4.1. The contracting laboratory must be comprehensive and able to perform as a minimum, all the tests in their guidebook. 4.2. Results must correlate with the clinical picture presented by the patient. Repeat analysis, at no charge to government must be provided on request in the event of a clinical disagreement. To provide this service, all specimens must be retained at least two weeks. 4.3. The contractor must be accredited by the College of American Pathologists (CAP) and participate in the CAP surveys programs. Quarterly results of the CAP surveys must be provided upon request at no charge to the government. Documentation of in-house quality assurance programs and data shall be provided to the 470th Air Base U.S Clinic Laboratory upon request at no charge to the government. 5. Other Services to be performed. 5.1. The contractor shall provide, at no charge to the government, tests on split samples for method evaluation. The 470th Air Base U.S Clinic Laboratory must notify the contractor in advance to identify these specimens. 5.2. Technical consultation services shall be available to discuss results and their significance with the 470th Air Base U.S Clinic medical staff. The service will be at no charge to the government and available to all clinicians and laboratory staff. 5.3 The contractor shall provide copies of their ?Laboratory Guide? giving normal ranges, specimen requirements, and result significance at no cost to the government. Lists of set-up times shall be provided upon request at no charge to the government. 5.4. Special shipping containers and/or special supplies must be provided at no cost to the government. 5.5. In the event the contractor receives an unacceptable specimen, the 470th Air Base U.S Clinic Laboratory must be notified within 24 hours via telephone. 5.6. The contractor must agree to notify the 470th Air Base U.S Clinic Laboratory of any price increase at least 60 days prior to the increase. 6. Acceptance of work being performed satisfactorily will be evidenced by the inspection activity named issuing a certificate of satisfactory performance. This certificate will be required before payment can be made. 6.2.
