Description
COMBINED SYNOPSIS FOR FLAG CASES 1. FISC Norfolk Philadelphia Contracts Division intends to procure Flag Cases, for the Navy Mortuary Affairs, Great Lakes, IL. Request for Quotation (RFQ) N00140-06-T-0700. FISC Philadelphia Contracts Division contemplates a firm fixed price contract. Delivery of supplies is required with 10 days after receipt of order. This is a combine synopsis/solicitation for commercial items prepared in accordance with Simplified Acquisition Procedures. This announcement constitutes the only solicitation; proposal being requested and written solicitation will not be issued. It is a Full and Open requirement, and it is the responsibility of contractors to familiar with the applicable clauses and provisions. The clauses can be access in full text at these addresses: www.arnet.gov/far and www.dtic.mil/dfars. The NACIS CODE IS 321920. Contract line item number(s) ITEM 0001 QTY 1000 Each ? Heritage, Solid Walnut Triangle Case w/Solid Walnut Wood Raised Panel Front Lid (Not Pressed Wood or Other Substitute). Overall Dimensions 25 3/8? x 12 ?? 4 3/8?, size for a 5x9 Foot Folded Burial Flag. Lid Hinged to open 105 Degrees W/2 Antique Brass Hinges. Walnut Back Panel (Not Pressed Wood or Other Substitute). Concealed Magnet Closure Triangular Velvet Insert with a Rigid Backing Suitable for Displaying Ribbons; Blue for navy, Red for Marine Corps 3? x 6? Black Brass Engraving Plate (Loose Indide Case) Wall Medallion Dimensions 2 ?? Diameter; Closed Case must Prevent Dust from Entering Case (Lid Must be Flush Against Case when Closed). Case Exterior Hand Rubbed and Waxed. This is Indefinite Delivery/Indefinite Quantity Order. Period of Performance: 01 Oct 2006 thru 30 Sept 2007. SPECIFICATIONS - Shipping and Handling Specifications ? Cases are to be packed individually according to industry standards to prevent any damage to the cases. Shipping will be accomplished by least costly method and/or quickest method of shipment to meet delivery terms via UPS, USPS, FEDEX. All cases must be mailed/shipped NLT 10 business days of order placed with Vendor. Notification of order will be verified to Mortuary Affairs by Email or fax. Overseas shipments of cases can be requested in those cases vendor will be notified of any additional shipping cost - Reshipment, Returns ? Returns or non-deliverable cases will be held by the vendor until the reason for non-delivery is established. If vendor send case to wrong address, re-shipment will be at no cost to Government. If MMSO provided incorrect information additional shipping charges will be paid. ? Invoice Submission ? Each order will be placed via the Wide Area Work Flow System upon notification of order. Vendor must be registered with WAWF and have a CCR registered to received payments through EFT means. No hard copies checks will be sent by U.S. Treasury. ? Mail Notification ? Vendor will send notification to MMSO NLT 14 Days post order that the Flag Cases were properly shipped and received by family member. Vendor will provide verification of delivery from the shipper that includes proof from the shipping company that delivery was accomplished. Access to shipping company?s data base with delivery receipt is sufficient. (ALL INTEREST PARTIES MUST BE CCR REGISTERED). The provisions at 52.212-1, Instructions to Offerors Commercial Items is applicable to this procurement. Offerors are advised to include a complete copy of the provisions at 52.212-5 with their offer. The clause 52.213-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The following clauses are applicable to this acquisition. 52-212-5 Contract Terms and Conditions Required to Implement Statues of Executive Orders, Commercial Items; 52.222-19 Child Labor Cooperation with Authorities and Remedies; 52.222-21 Prohibition or Segregated Facilities; 52.222-26 Equal Opportunity; 52.232-33 Payment for Electronic Funds Transfer-Central Contractor Registration; 52.249-1 Termination for Convenience of the Government (Fixed Price); 52.212-2 Evaluation ? Commercial Items is applicable to this procurement. While price will be a significant factor in the evaluation of offers, the final contract award will be based on a combination of factors ? combination of price and past performance. The quoter shall submit, as part of its quote information on previously performed contracts or on-going contracts the solicitation performed for Federal, State, or Local Governments, and for commercial firms. Information shall be provided on your most relevant and will not exceed three sources. The information submitted shall include a contract number, the total dollar value, and period of performance of contract and POC knowledgeable about your performance under that contract.
