Offers shall consist of the following: 1.1 A cover letter that (a) states whether the offeror proposes to deliver the requested items to the address identified in block 15 within seven (7) days of award; (b) identifies the solicitation number; (c) the time specified for receipt of offers; (d) the name, address, and telephone number of the offeror; (e) terms of any express warranty; (f) any discount terms; (g) “remit to” address, if different than mailing address; (h) past performance information that identifies three contracts/agreements that demonstrate past performance relevant to the solicitation requirements with the contract number, brief description of services/supplies, name, telephone number, and email address of a person responsible for administering the contracts/agreements; (i) acknowledgement of solicitation amendments (if applicable). 1.2 The unit price and total price for each of the CLINS above. Delivery is "FOB Destination" and thus total price quoted shall include freight charges for delivery to the address specified above. 1.3 Representations and Certifications. If the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov, the offeror shall complete only paragraph (j) of provision 52.212-3 Offeror Representations and Certifications—Commercial Items. If the offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision. 2. The Government will award a contract resulting from this solicitation to the lowest priced, responsive, responsible offeror whose offer meets or exceeds the acceptability standards for non-cost factors. The non-cost factors are as follows: 2.1 Proposed Delivery: The offeror __ does or __ does not propose to deliver the requested items to the address identified in block 15 of the solicitation within fifteen (15) days of award or sooner. 2.2 Past Performance: The offeror’s demonstrated record, as confirmed by references, of successful past performance of the same or substantially similar contract efforts __ is or __ is not acceptable.
