- Department of Agriculture, animal and Plant Health Inspection Service, Veterinary Services, 1800 Dayton Avenue, AMES, IA, 50010 This is a combined synopsis/solicitation for commercial supplies prepared in accordance with Federal Acquisition Regulation (FAR) 12.6, and supplemental information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will note issued. This combined synopsis/solicitation will result in a firm fixed price contract. Simplified procurement procedures will be used as well as commercial item acquisition regulations per FAR12. The NAICS code is 424990. This requirement is for the United States Department of Agriculture (USDA), Animal Plant Health Inspection Service (APHIS), National Veterinary Services Laboratories, 1800 Dayton Ave., Ames, IA 50010. The reference number for this effort is FBO-MB33832 and this combined synopsis/solicitation is issued as a request for quotation (FRQ). The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. Requirements and Quantities: This requirement consists of Becton-Dickinson products or equal to be shipped on a monthly basis as indicated. Selection will be based on low price and more than one bid may be acceptable. Following are the requirements: (1) 221414 Mycobactosel LJ slants, 6 ctns/mo, (2) 221958 7H11 w/aspartic acid and pyruvate, 45 ctns/mo, (3) 220909 LJ slants, 2ctns/100/mo, (4) 221257 LJ deeps, 2 pkg/10/mo, (5) 221896 LJ slants w/5% NcCL, 3pkg/10/mo, (6) 297206 LJ slants w/iron 3 pkg/10/mo, (7) 222233 HEYA w/MJ and ANV (2lots per shipment) 4 ctns/mo, (8) 245122 Bactec MGIT bar-coded 7ml tubes 5 ctn/100/mo, (9) 245124 Bactec MGIT 960 supplement kit, 3 pkg/100 tests/mo, (10) 245154 Bactec MGIT 960 ParaTB medium, 1 ctn/100/mo, (11) 245156 Bactec MGIT 960 ParaTB supplement kit 1 pkg/6x15ml/mo. The period of performance for this contract is from nOVEMBER 01, 2006 or date of award whichever is later. The provision at 52.212-1 Instruction of Offerors ? Commercial applies to this solicitation. The provision at 52.212-2 Evaluation ? Commercial Items applies to this solicitation. Award will be made to the responsible offeror whose offer conforming to this combined synopsis/solicitation will be most advantageous to the Government, price, and other factors considered. The Government intends to evaluate offers and award without discussion, however, the Government reserves the right to conduct discussions later if determined necessary by the Contracting Officer. The following factors shall be used to evaluate offers: Price and past performance. Past performances a measure of the degree to which an offeror has satisfied customers in the past, and complied with federal, state, and local laws and regulations. The assessment will consider the firm?s record of conforming to specifications and to standards of good workmanship/customer service, and adherence to contract schedules, terms and conditions. The provision 52.212-3 Offeror Representations and Certifications ? Commercial Items applies to this solicitation. The contractor shall either complete the on-line Offeror Representations and Certifications or return a completed copy of the Offeror Representations and Certifications with their quotation.
