Description
Description: This is a commercial item acquisition conducted in accordance with Federal Acquisition Regulation (FAR) Part 12. This acquisition is for the transport of a mobile PET/CT scanner. The Miami VA Healthcare System intends to award a Fixed-Price Requirements contract to a licensed, bonded transporter. The unit shall be transported between the West Palm Beach VA Medical Center 7305 N. Military Trail, West Palm Beach, FL and the Miami VA Medical Center 1201 NW 16th Street, Miami, FL twice weekly. Contractor shall furnish all equipment, supplies, and materials necessary for the transportation and set-up as outlined in the Description/Specification/Work Statement. Offers shall be considered only from professional transport companies whose driver(s) is duly licensed to transport radioactive materials and trained in related Department of Transportation (DOT) regulations. Contractor must have insurance coverage of no less than $2.5 million dollars to cover damage to the unit and property during transit plus no less than $1.0 million dollars of additional liability insurance. All contractor employees doing business with the Department of Veterans Affairs shall be the subject of a background investigation. The contract period will consist of a base period of one (1) year with the option to extend the contract for four (4) years. This procurement will be 100% set-aside for Small Business, NAICS Code 484220, and size standard of $23.5 mil. The offeror is responsible for downloading all amendments and other documents from the BOS website without further notices from the Department of Veterans Affairs, Miami VA Health Care System, Miami, FL. Proposals shall be submitted by November 1, 2006 at 4:00 pm (EDT) either by mail or handcarried. Fax/E-mail proposals shall not be accepted.
