- The Carl T. Hayden VAMC, Phoenix, Arizona, is seeking to award multiple basic ordering agreements for home and respite care hospice healthcare services within the catchment area of the Carl T. Hayden VA Medical Center, Phoenix, AZ 85012-1892 which encompasses Maricopa County and Gila County, Arizona, for a base period of October XX, 2006 through October 30, 2007 with four option years ending October 30, 2011, per the performance requirements in the solicitation VA-644-07-RP-0006. The North American Industry Classification System Number (NAICS) for the services is 621610 and the Business Size Standard is $12,500,000.00. Prospective offerors must be Medicare certified hospice care agencies. The solicitation will be made available on or about October 17, 2006, with a due date of not later than October 30, 2006, by 3:00PM US/Arizona MST but offers will be accepted sooner. The VA intends to award multiple basic ordering agreements to allow patients reasonable choices for hospice care. Copies of the solicitation may be obtained at no charge at the Federal Business Opportunities website at http://www.fedbizopps.gov/ Dissemination is limited to electronic medium and the VA will provide no hard copies. Any amendments issued to this solicitation will also be available at the URL address. This solicitation is unrestricted full and open competition.
