- This announcement constitutes a Sources Sought Synopsis and Request for Information. The information requested will be utilized to assist the Navy in conducting market research of industry to identify only HUBZone small business sources that may successfully provide these services to the Government. The Navy intends to release a Request for Proposal (RFP) for the provision of Food Services for the Naval Hospital Pensacola. The draft Scope of Work is as follows: 1.1 SCOPE OF WORK 1.1.1 The Contractor shall furnish all labor, supervision, transportation, equipment, food and supplies except as specified herein as Government-furnished to provide Medical Food Services and dining room food service described in Section C-5 on a regular schedule as specified in Section C-7 for Naval Hospital Pensacola, hereafter referred to as the MTF. 1.1.2 Contractor responsibilities include, but are not limited to, the following: 1.1.2.1 Provide sufficient qualified personnel to perform the services specified in this contract. 1.1.2.2 Requisition, purchase, receive, transport, store and control subsistence, consumable supplies and equipment. 1.1.2.3 Prepare and serve cafeteria meals for all authorized persons entering the dining facility. The Government shall perform verification of entitlement to use the dining facility and collect funds due for cash sale of meals. 1.1.2.4 Prepare regular and therapeutic diet meals and nourishments for all inpatients and deliver same to Nursing units of the MTF. 1.1.2.5 Operate the scullery and ensure that cookware, hand tools, and tray service equipment is maintained in a sanitary state. 1.1.2.6 Provide sanitation and housekeeping services in food storage, preparation, service, dining areas, scullery, and employee locker rooms and lavatories. 1.1.2.7 Maintain the security of both Government and Contractor furnished supplies, equipment, furnishings and facilities. 1.1.2.8 Maintain specific records and provide reports as required by this contract. 1.1.3 The Contractor shall provide supplies and equipment, including preventative maintenance, as described in Section C-4. 1.1.4 It is essential that continuity of services be maintained to the maximum degree possible; therefore, substitution of contract employees shall be kept to the absolute minimum necessary to perform the services required and to provide adequate back-up personnel. 1.2 INDEPENDENT CONTRACTOR. The services rendered by the Contractor are rendered in the capacity of an independent contractor. The Government will evaluate the quality of both professional and administrative services for the purpose of contract inspection and acceptance. The Government retains no direct control over professional assessments and/or recommendations. The Contractor shall be solely responsible for any and all liability caused by the acts or omissions of its agents or employees. The Contractor shall not in any manner represent or infer that it is an instrumentality or agent of the United States Government. The Contractor shall recognize that the Commanding Officer maintains administrative and operational responsibility for all activities within the command and will take such actions as necessary to preserve and maintain the integrity of the command, subject to the limitations prescribed by law and U. S. Navy Regulations. 1.3 MODIFICATIONS: The Contracting Officer will designate and authorize an individual to act as the Contracting Officer?s Representative (COR). Any such representative appointed will be specifically designated by letter from the Contracting Officer. The COR exclusively represents the Contracting Officer in all technical phases of the work, but is not authorized to issue Change Orders, Supplemental Agreements, or direct any contract performance requiring contractual modification or adjustment. Changes to the contract can only be made by modification properly executed by the Contracting Officer. All observations made by persons other than the Contracting Officer or the COR are strictly advisory and shall not influence the Contractor?s operations except for administrative requirements and responsibilities specified herein. 1.4 BACKGROUND INFORMATION: This is a nonpersonal services contract covering a professional discipline which provides healthcare services to eligible beneficiaries of the Department of Defense direct care system. The system comprises service-owned, staffed and operated hospitals and clinics throughout the world. Contracting has been chosen as a strategy to permit expansion of the healthcare capacity of DOD facilities by overcoming shortfalls in the numbers of uniformed practitioners and support personnel. 1.5 QUALITY OF SERVICE: The services specified in this work statement shall be performed in accordance with established principles and ethics of the medical profession. The quality of healthcare provided will meet or exceed the current recognized standards established by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). The Contractor shall comply with applicable provisions of law and the rules and regulations of any and all Governmental authorities pertaining to regulation of food service personnel and medical treatment facilities, the regulations and standards of the MTF and the bylaws of the hospital?s medical staff. The Contractor shall adhere to and comply with all Department of the Navy, Bureau of Medicine and Surgery and Naval Hospital Pensacola Instructions and notices which may be in effect during the term of the contract. In all cases, the dignity of the patient will be given the highest regard, and the precepts of the American Hospital Association?s ?Bill of Rights for Patients? shall be observed. To achieve these goals, the Contractor shall establish, submit for Government approval and implement a quality control program which is compatible with the MTF Quality Control Program. The Contractor shall also actively participate in the established MTF Quality Improvement Program. Current copies of both programs shall be maintained on-site for ready reference by the contract food service workers during the term of the contract. 1.5.1 Quality Control (QC) Program 1.5.1.1 After contract award and not later than commencement of services, the Contractor shall submit its Quality Control Program to the COR and to the Contracting Officer for approval. The program shall be periodically updated when requested by the COR. 1.5.1.2 The Quality Control program shall, at a minimum, include: 1.5.1.2.1 All services listed shall be inspected by the Contractor using random sampling except sanitation inspections which shall be performed by the Government twice a month accompanied by the Project Manager or designated representative (only if the Project Manager is not present). All other inspections shall be performed by the Project Manager or designated representative. 1.5.1.2.2 A file of inspections conducted and corrective action taken shall be maintained by the Project Manager which shall be kept in the Food Service Manager?s Office and shall be made available to the COR upon request. 1.5.1.2.3 Written Instructions and Procedures: The contractor?s Quality Control Program shall include written work instructions and procedures, typically named Standard Operating Procedures, to complete the work specified in this contract. These work instructions and procedures shall comply with the current standards established by the JCAHO for Nutritional Services. All standards and required characteristics listed in the JCAHO manual directly applicable to this Food Service contract shall be referenced by the contractor?s Standard Operating Procedures. The COR shall be on the document distribution list for all formal changes to contractor?s written instructions and procedures. 1.5.1.2.3.1 The contractor shall establish Quality Control Mechanisms as specified in the current JCAHO Accreditation Manual for Hospitals. The contractor shall have documented subject, method of review, frequency of review, data collection, findings, and a plan of action for each Quality Control Mechanism (QCM) reviewed. The QCMs findings and plans of action will be reviewed by the COR/QAE monthly. 1.5.1.2.4 Inspection System: The contractor?s Quality Control Program shall include an inspection system covering the services listed in this Performance Work Statement. The contractor shall establish an assessment plan (e.g., survey instruments and audit protocols) that ensure compliance by contractor employees with contractor developed Standard Operating Procedures. An assessment of revised operating procedures and instructions shall be retained throughout the life of this contract. These assessment files shall be furnished to the COR for review, as requested or when required by inspecting officials, such as the JCAHO or Inspector General, Navy. 1.5.1.2.5 Method of Detection: The contractor?s Quality Control Program shall include a method of early detection and correction of conditions adversely affecting the quality of service, to include procedures for detecting discrepancies (e.g. serving temperatures, patient or patron complaints) and causes of defects and action plans to correct recognized discrepancies. This method shall include providing timely written explanation of the correction of the defects and correction of cause in response to a Government corrective action request (e.g., DD Form 1715) and shall also include bacteriological monitoring, when necessary. 1.5.1.2.6 Maintenance Procedures: The contractor?s Quality Control Program shall include instructions for preventative maintenance and use of inspection records and assessment documentation. Records are considered one of the principal forms of objective evidence of current and past quality control surveillance. The contractor?s Quality Control Program shall ensure that Contractor records are complete, accurate, and conform to the current requirements of the JCAHO. 1.5.1.2.7 Management Action Plan: The contractor?s Quality Control Program shall include a method for analysis of inspection findings, including Contractor and Government records, customer complaints, and assessment results, and for development of corrective management action plans, with follow-up after implementation of corrective plan. 1.5.1.3 Upon receipt of the Contractor?s Quality Control Program, the MTF will have 10 working days to review, comment, approve and/or disapprove. If the MTF finds deficiencies in any portion or portions of the proposed program, the Contractor shall have 10 working days to correct the deficiencies. This review and correction process will continue until the entire proposed program is approved in writing by the MTF. 1.5.2 The Contractor shall maintain a copy of this contract, the Contractor QC plan and MTF QI plan on-site at all times. Contract employees shall read, be familiar with, and implement the contract, including the technical proposal, Quality Control Program, MTFQuality Improvement Program and any revisions thereto. Each experience summary should include: (a) name of the project; (b) brief description of the project; (c) contract or project number; (d) customer point of contact (name, address, phone); and (e)dollar value of the project; and (5) A statement regarding if your company intends to perform the majority of the contract requirements, or will be teaming with another company. If more than one firm is contemplated, please identify the socioeconomic status of each. Responses should be submitted to C.
