- The National Institute of Child Health and Human Development has a requirement to conduct a study of program interventions designed to enhance carbohydrate quality through increased consumption of nutrient-dense whole foods in youth with Type 1 diabetes and youth with or at risk for Type 2 diabetes. This requirement will consist of PART A and PART B. Offerors may respond to either PART A or PART B or both PART A and PART B. PART A: Enhancing Carbohydrate Quality in Management of Type 1 Diabetes -- This study will test an innovative program of interventions designed to promote consumption of carbohydrates from nutrient-dense whole foods and to determine the efficacy of such a dietary change in promoting improved health outcomes among children and adolescents with type 1 diabetes. Study participants will be children and adolescents with type 1 diabetes identified and recruited through current clinical care. Objectives include the assessment of (1) the efficacy of behavioral intervention in promoting long-term change in dietary practices, and social and cognitive mediators of dietary practices; (2) feasibility of increasing consumption of fruits, vegetables, whole grains, and legumes among free-living youth with diabetes; and (3) the short- and long-term efficacy of these dietary practices in promoting improved glycemic control, improved body composition, and other indicators of health status. Additional research questions include (1) parent and child social and cognitive factors associated with consumption of fruit, vegetables, whole grains, and legumes; (2) the contribution of foods and food groups to overall carbohydrate consumption, including developmental variability; and among those diagnosed with diabetes, (3) the influence of carbohydrate quality and dietary glycemic index on insulin dose required, and (4) the influence of the dietary intervention and subsequent dietary change on quality of life. PART B: Enhancing Carbohydrate Quality in Management and Prevention of Type 2 Diabetes -- This study will test an innovative program of interventions designed to promote consumption of carbohydrates from nutrient-dense whole foods and to determine the efficacy of such a dietary change in promoting improved health outcomes among children and adolescents with or at risk for type 2 diabetes. Study participants will be children and adolescents identified and recruited through current clinical care who are diagnosed with type 2 diabetes or at high risk based on BMI and family history or metabolic parameters. Objectives include the assessment of (1) the efficacy of behavioral intervention in promoting long-term change in dietary practices, and social and cognitive mediators of dietary practices; (2) feasibility of increasing consumption of fruits, vegetables, whole grains, and legumes among free-living youth with or at risk for type 2 diabetes; and (3) the short- and long-term efficacy of these dietary practices in promoting improved glycemic control, reduced insulin resistance, improved body composition, and other indicators of health status. Additional research questions include (1) parent and child social and cognitive factors associated with consumption of fruit, vegetables, whole grains, and legumes; (2) the contribution of foods and food groups to overall carbohydrate consumption, including developmental variability; and among those diagnosed with diabetes, (3) the influence of carbohydrate source on medication dose required; and (4) the influence of the dietary intervention and subsequent dietary change on quality of life. It is anticipated that one to two cost-reimbursement, completion type contracts will be awarded, each with a period of performance of 5 years, beginning approximately April 15, 2007.
