Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W91YTV-07-T-0006 is issued as a Request for Quote (RFQ). This acquisition is issued as an Unrestricted Open Market Procurement to obtain the maximum amount of competit ion from all responsible business concerns. The North American Industry Classification System Code (NAICS) is 811219 with small business size standard annual revenue of $6 million dollars. The Standard Industrial Classification Code (SIC) is 7699. This requirement is for Annual Maintenance Service on (3) ADC Compact Digitizer Imaging Systems inclusive of hardware, firmware and software; the subject equipment is government owned medical equipment. Please quote pricing based on issuance of a Multi-year Co ntract with a Base Year plus 4 One Year Option Periods: Base Year :1 Nov 2006 -30 Sep 2007 $ ____________ Option Year 1: 1 Oct 2007 30 Sep 2008 $__________ Option Year 2: 1 Oct 2008 30 Sep 2009 $___________ Option Year 3: 1 Oct 2009 30 Sep 2010 $___________ Option Year 4: 1 Oct 2010 30 Sept 2011$ __________.
