- Position: DENTAL HYGIENIST Department: Dental Location: Crow Service Unit, Crow Agency, Montana Term: November 20, 2006 thru September 30, 2007. Opening Date: October 30, 2006 Closing Date: November 06, 2006 Contractor is sought to serve as a Dental Hygienist at the Indian Health Service Hospital at the Crow/Northern Cheyenne Hospital, Crow Agency, Montana. The purpose of this position is to provide dental hygiene care to pediatric, adolescent, adult and geriatric patients. Contractor will perform duties as outlined in contract and Statement of Work. Contractor's tour of duty will run two (2) Days a week. One (1) Day at the Crow/Northern Cheyenne Hospital and One (1) Day at the Lodge Grass Clinic. 8:00 am through 4:30 pm. The Crow Service Unit Chief of the Crow Dental Program will supervise the contractor. Contractor will be paid for hours worked with no paid holidays, no paid leave, and no fringe benefits. Basic Education Requirements: 1. Successful completion of a full four year course of study in an accredited college or university leading to a Bachelor's degree which must have included or been supplemented by successful completion of a full academic curriculum in dental hygiene accredited by the American Dental Association's commission on dental accreditation, or; 2. Experience in an amount which, in combination with successful completion of a full academic curriculum in dental hygiene, accredited by the American Dental Association's commission on dental accreditation, is sufficient to total at least four years of experience and education. Appropriate experience may be equated to education on a year-for-year basis. Applicants must submit the following information: 1. Resume to include education, experience and certifications. 2. Salary Indication - Indicate the wage you are willing to work for - for each day of clinic. 5. BIA Form 4432/Certificate of Degree Of Indian Blood (If Claiming Indian Preference) Award will be made on the basis of price and other factors representing the best value to the Government. Failure to submit all required documentation with your application will result in your application being incomplete.
