Description
This announcement constitutes the only solicitation; a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional inf ormation included in this notice. This solicitation is issued as a Request for Quotation (RFQ). The North American Industrial Classification System Code (NAICS) is 238290, and the size standard is $13 Million. This procurement is a 100% set-aside for Sm all Businesses. The pricing must be quoted as follows: Contract Line Item 0001  (Service) Perform Semi Annual Maintenance on Hangar Doors for the time frame of January 1 through December 31, 2007. The Contractor is to furnish all plant, labor, material and equipment necessary to perform semi annual maintenanc e and inspection to 22 vertical hangar doors located at Building P-2060, P-2070, P-2072 and P-2074 at Fort Drum, NY 13602. 0001 Hangar Door Maintenance Quantity Unit Unit Price Amount 44 Each $______ $______ Contract Line Item 0002- (Service) Perform Quarterly Maintenance on 2 canopy (tip-up) hangar doors for the time frame of January 1 through December 31, 2007. The Contractor is to furnish all plant, labor, material and equipment necessary to perform quart erly maintenance and inspection to 2 canopy doors located at Building P-2049 at Fort Drum, NY 13602. 0002 Hangar Door Maintenance Quantity Unit Unit Price Amount 8 Each $_______ $______ Contract Line Item 0003- (Service) Perform Quarterly Maintenance on hangar doors for the time frame of January 1 through December 31, 2007. The Contractor is to furnish all plant, labor, material, and equipment necessary to perform semi annual maintenance and inspection to 3 vertical doors located at Building P-2050 at Fort Drum, NY 13602. 0003 Hangar Door Maintenance Quantity Unit Unit Price Amount 12 Each $_______ $______ Contract Line Item 0004 - Contractor Manpower Reporting (CMR). Contractor must report man-hours in accordance with guidelines in the PWS. Reporting must be done before invoicing. Please quote the cost of CMR if any. In no event shall failure to inspect the site constitute grounds for a claim after contract award. The following DFARS clauses, cited within 252.212-7001, are applicable to this acquisition: 252.225-7001 and 252.232-7003. DEPARTMENT OF LABOR WAGE DECISION. The Service Contract Act of 1965 will apply to this service, the U.S. Department of Labor Wage Determination number 2005-2377, Revision No. 1, dated 09/20/2006, for New York State, Jefferson County, will apply to this se rvice. Past Performance Information. In accordance with paragraph (b)(10) of the basic clause (52.212-1 Instructions to Offerors--Commercial Items) provide past performance information to include; recent and relevant contracts within the past three years for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information). Note: Completion of the CCR and ORCA data entry are mandatory requirements for award, failure of interested parties to complete these requirements will be deemed to be non-responsive and thereby make the offeror ineligible for award. Offerors are also req uired to fill out and return FAR Provision 52.204-8 with the offer.
