- 311 HSW AFIOH/SDE BIOHAZARDOUS WASTE PICK UP STATEMENT OF WORK 15 November 2006 Period of Performance (POP) 15 Dec 2006 -- 14 Dec 2007 1. SCOPE: The Contractor shall provide AFIOH/SDE (Epidemiological Surveillance Division), Bldg 930, Brooks City-Base TX 78235-5132 with all labor, equipment materials, packing containers, vehicles and management services necessary to accomplish the collection, sorting, packaging, manifesting, transportation and incineration of items designated as biohazardous. 2. COMPLIANCE WITH STATE AND FEDERAL STANDARDS: All work must be accomplished in accordance with applicable Federal, State and Local Regulations, including the Department of Transportation (DOT), Office of Safety and Health Administration (OSHA) and Environmental Protection Agency (EPA); see Paragraph 10. 3. PICK UP: Pickup of all biohazardous waste, including sharps, will occur weekly or bi-weekly, depending on need (chemicals or chemotherapeutic agents are EXCLUDED from this pickup). Annual projection is for 1,610 containers per year, or approximately 31 containers per week. Containers are 3.1 cubic feet and maximum weight is 45 pounds per container. The Contractor is to report to the Primary or Alternate Technical Program Manager in Bldg 930 for pickup of boxes (see Paragraph 11 for Points of Contact). 4. PACKING PROCEDURES: Contractor will supply boxes with three millimeter liners which meet or exceed all Federal, State and Local guidelines. AFIOH/SDE personnel will place biohazardous waste materials in the boxes and seal boxes when full. Contractor will document the following information on the sealed boxes: Transporter Generator Manifest Number 5. MANIFESTING: The Contractor shall prepare a uniformed medical waste management manifest for each transportation container in accordance with applicable regulations. An AFIOH/SDE Representative and the Contractor/Transporter shall sign the manifest before the waste leaves Brooks City-Base. The AFIOH/SDE Representative shall be given the Generator?s first copy of the manifest. A manifest certifying final transportation of waste must be received at AFIOH/SDE, Bldg 930, 2730 Louis Bauer Dr, Brooks City-Base TX 78235-51 32 within 30 days from the date the waste was removed from AFIOH/SDE (Bldg 930) by Contractor personnel. A copy of the manifest will be maintained by the responsible section (s). 6. TRANSPORTATION: The Contractor shall provide all labor, equipment and vehicles for transporting the manifested medical sharps from AFIOH/SDE to an appropriate civilian treatment disposal facility. All transporters shall have an EPA Identification Number, which shall be provided upon request. Waste shall be moved in the Contractors vehicle to the Contractors disposal facilities. The vehicle shall be suitable for the task of safely transporting medical sharps. 7. ACCIDENTS/SPILLS: The Contractor shall be responsible for providing all clean-up services and materials for medical waste spilled or leaked while in their possession, from point of pickup to the disposal site. 8. TREATMENT/DISPOSAL: The Contractor shall guarantee incineration of all biohazards waste removed from AFIOH/SDE in accordance with applicable Federal State and Local regulations. All storage or disposal sites shall be permitted under RCRA 40 CFR 264. A copy of the permit shall be provided before award of this contract. 9. ADDITIONAL REQUIREMENTS: The Contractor is responsible for having all applicable licenses and permits to perform the work required in this contract. Work Plan and copies of all such permits and licenses shall be current. Personnel, subcontractors and facilities must be qualified under applicable regulations and a summary of their qualifications must be submitted upon request. The contractor shall allow representatives of AFIOH/SDE and/or Brooks City-Base Environmental Management Directorate to visit any of its own or subcontractor?s facilities at any time during the contract period to ensure that the requirements set forth are met. 10.
