- Provide Pharmacist services at Ireland Army Community Hospital (IACH), Fort Knox, Kentucky. during the period of 11 December 2006 through 10 December 2007 with one (1) one-year option periods. The contract pharmacist shall be a graduate of an accre dited college of pharmacy. The contract pharmacist shall have and maintain a valid unrestricted state license in any state in the United States, District of Columbia or the territory of Puerto Rico. The pharmacist shall not have a history of license revo cation, suspension or encumbrance due to misuse or improper dispensing of controlled substances. The contract pharmacist shall have a minimum of 4 years of retail or outpatient pharmacy experience. This requirement is unrestricted, NAICS 561320, with a size standard of $12.5M, applies to this requirement. The solicitation will be issued on or about 6 December 2006 with quotes due on or about 27 December 2006. The solicitation will be posted on the Fort Knox homepage website at http://doc.knox.army.m il on or about 6 December 2006. No paper copies of the solicitation will be issued. All amendments to the solicitation will be likewise posted to this website. Contractors are cautioned to check this website for amendments. No solicitation mailing list will be compiled.
