Description
Sources sought to provide Cardiac Event Monitoring Services for the VAMC Spokane, WA. DESCRIPTION OF SERVICES: The Spokane VAMC requires approximately 10 event monitors per month to be used by our veteran population. Additional monitors may be needed during the contract period at times of high usage. In such situations, the contractor will provide sufficient monitors to meet our ongoing needs. Monitors will be kept on site at the Spokane VAMC and will be issued by Diagnostic Imaging Service (Vascular). The POC for the VA is Mike Ricks, Health Technician (509) 434-7339. Patients will use the event monitor for approximately 30 days before returning them to the Contractor or Spokane VAMC using a postage-paid return container supplied by the Contractor. At the end of the 30 day monitoring period the patient will return the monitor to the Contractor or VA who in turn will return it to the contractor. Upon receipt of the monitor, the Contractor will then issue a replacement monitor. All patient supplies shall be included in the contract (e.g. monitor, holder, ECG electrodes, batteries, pre-postage paid shipping container, etc). The NAICS code for the procurement is 621999 and the small business size standard is $9 million. Your response should identify your company name, point of contact with contact information, indicate all areas you are capable of providing service, capabilities statement and identify your business size, and if you are (by the definitions in the FAR) any of the following: Small Business, 8(a) disadvantaged, disadvantaged, woman-owned, veteran-owned, service disabled veteran owned or HUB Zone certified. All responses shall be submitted no later than 3:00 PM PDT, December 22, 2006 to the Contracting Officer, Rich Hague at richard.hague@va.gov. Only this minimal information is requested. A Request for Quote shall be issued at a later date.
