Description

The Great Lakes Acquisition Center (GLAC) is soliciting firm fixed price quotations to provide Hemodialysis services for the Department of Veterans Affairs Medical Center, 324 East ?H? Street, Iron Mountain, Michigan. Due to medical necessity with regard to the patients? conditions, the prospective contractor shall be capable of providing hemodialysis services within a 50-mile radius of the VA. Anticipated period of performance is one year from date of award with four additional one-year option years. Anticipated award date is January 1, 2007. Electronic version and hard copy of solicitation will be available on or about November 30, 2006. This procurement is being conducted under the Subpart 13.5 test program in accordance with Parts 12, 13, or 15 of the FAR or some combination thereof. NAICS number is 621492 with a small business size standard of $31.5 million. Electronic solicitation may be obtained FREE at the following URL: http://www.fedbizopps.gov. Requests for hard copy solicitation must be in writing to the GLAC Contact Point and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
