- This is a combined synopsis/solicitation for a Brand Name commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for Midmark Cabinetry for the oral surgery clinic. Quotations are being requested and a written solicitation will not be issued. Solicitation FA7000-07-T-0099 is issued as a Request for Quotation (RFQ) for Brand Name only operating room Midmark Cabinetry. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Fac 2005-13, Effective 28 Sep 2006. This acquisition is full and open. NAICS code for this solicitation is 423450 -- Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers. This RFQ is being conducted as a commercial item procurement using Simplified Acquisition Procedures (FAR Part 13). No paper copies of this solicitation will be provided. If providing an equal item please provide specifications and/or pictures. Product must include detailed specifications of cabinet structure and material, storage in reference to the attached drawings. Quote shall state, GSA number (if applicable) and tax ID number. Qoute must be FOB destination for shipping/delivery. This RFQ contains eleven (11) line items. Quotations are being solicited on Brand Name OR EQUAL in accordance with FAR 52.211-6. Evaluation factors are price, technical, and delivery. Responding quotes shall include detailed technical information to enable the government to make a complete evaluation and appraisal of the offer with respect to the requirements set forth in this combined synopsis solicitation. No site visit is required. Cabinet evaluation factors shall be; non-porous, steel construction, electrostatic powder coat, sealed baked on paint, steel frame, scratch resistant finish, seamless polymer-cover panels that is spill, fluid and chemical resistant with no surface degradation. Seamless polymer-drawers shall be of one solid piece construction with rounded corners for ease of maintenance and pill containment. The Contracting Officer will evaluate "equal" products on the basis of the evaluation factors listed in this solicitation and the information furnished by the offeror identified in the quotation and reasonably available to the Contracting Officer. THE CONTRACTING OFFICER IS NOT RESPONSIBLE FOR LOCATING OR OBTAINING ANY INFORMATION NOT IDENTIFIED IN THE QUOTATION. Offerors shall include manufacturer's name, model number, and specifications in your quotation. Offers? shall clearly indicates their quote is being offered as an "equal", or brand name product referenced in this solicitation. Prospective offerors must be registered in the Central Contractor Registration (CCR) http://www.ccr.gov or by calling 1-888-227-2423. Offerors? shall complete the Online Representations and Certifications Application (ORCA) at http://www.bpn.gov in order to be eligible for award. Information on annual Representations and Certifications can be obtained via the Internet at http://orca.bpn.gov. Late offers: -Any offers, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is ?late? and will not be considered. Offers must be received NLT 3:00 PM Mountain Daylight Savings Time, 01 Dec 2006, at 10th ABW/LGCB, Operational Contracting Office, 8110 Industrial Drive, Suite 200, USAF Academy, CO 80840-2315. Offers received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated.
