- REQUEST FOR INFORMATION Peripheral Vascular Stents.  The following is a request for information (RFI) in support of the VISN 1 Surgery and Radiology Programs in the VA New England Healthcare System.  This is a market survey for planning purposes only and should not be construed as a request for proposal/quote or as an obligation on the part of the government.  No reimbursement will be made for any costs associated with providing information in response to this announcement and/or any follow-up information requests.  The Government is seeking information on qualified vendors who are capable of providing coronary stents of all sizes that are of the highest quality and effectiveness.  All potential sources with the capability to supply peripheral vascular stents, biliary, balloon expandable, self-expanding, and covered self-expanding, are invited to submit a capability statement that is responsive to the government questions contained in this document.  The vendor's response to this RFI must address the government's specification questions, and provide individual and quantity pricing.  Additionally, the vendor is requested to submit questions, concerns, and any potential issues that may assist the government in developing a future requirement for the different types of coronary stents.  Replies to this RFI are not mandatory.  Replies will be separated from, and have no bearing on, subsequent evaluations of submission submitted in response to any resulting formal Request for Quote.  The use of information received from industry to complete final requirement and resulting solicitation will be at the discretion of the government.  Eligibility in participating in a future acquisition does not depend upon a response to this notice.  The Government will not critique a potential offer's information.  Proprietary information is not being solicited. Information considered proprietary, if any, should be identified as such. Potential Requirement: The on-site, immediate availability of a full-range of biliary, balloon expandable, self-expandable, and covered self-expanding peripheral vascular stents to provide timely, high quality and effective stenting is required.  Based on appropriate clinical criteria, physicians will use peripheral vascular stents to assist patients with peripheral vascular disease.  The physician will determine the use of biliary, balloon expandable, self-expandable, and covered self-expanding peripheral vascular stents during the patient's procedure. Subsequent patient procedures will follow the same protocols and will require the full-range of peripheral vascular stents. The Vendor MUST submit the response to this request for information, in accordance with the following government specification - 8 areas to be addressed.     1. Identify the peripheral vascular stents that will be provided, with the type of technology (e.g. biliary vs. balloon expandable vs. self-expandable vs. covered self-expanding) and the size (e.g. diameter and length). 2. What is the products Shelf life?  Government Provided Information:  The peripheral vascular stents will be stored for a period of time. Shelf Life must meet or exceed established national standards.     3. What is the Shipping time for the product? Government Provided Information:  The peripheral vascular stents will be sent to the VISN 1 Surgery and/or Radiology Departments in the VA New England Healthcare System. 4. What is your policy regarding consignment? Government Provided Information:  The peripheral vascular stents will be utilized on a regular basis in the VISN 1 Surgery and Radiology Programs. Inventory levels must be maintained at a level appropriate for the full-range of peripheral vascular procedures, and peripheral vascular stents at or near expiration at the facility will be exchanged for new products. 5. What is the products minimum/maximum storage temperature threshold? Government Provided Information:  The peripheral vascular stents will be stored in the VISN 1 Surgery and Radiology Programs in a location that experience some power fluctuations and have limited storage capability.  The peripheral vascular stents must be able to withstand temperature extremes above and below room temperature.     6. Is there additional Equipment required for your peripheral vascular stents? Government Provided Information:  Specify any additional equipment that may be necessary for use of the peripheral vascular stents. 7. Pricing: Provide the price per unit of the product for biliary, balloon expandable, self-expandable, and covered self-expanding peripheral vascular stents and specify any quantity discounts offered.  Government Provided Information:  The government has not determined the quantity required. 8. Provide socioeconomic status. Contemplated Type of Contract: Firm Fixed Price. Responses to this RFI must be received not later than January 5, 2007.  Submission is limited to 5 pages. All documents and materials submitted in response to this RFI will become the property of the government, and will not be returned. Submission requirement is electronically, via email to Alex.Dowgiallo@va.gov and james.adamo@va.gov. Responses will be marked as follows: "RFI VA-650-07-RI-0012".  Include with the capability response:  Vendor company name, address, name of the point of contact, email address, phone number and fax number, and vendor's DUNS number. If the offeror has a GSA FSS schedule, the vendor is to provide the GSA schedule number.  Questions concerning this RFI are to be submitted in writing within the response period allotted to this RFI.   Questions submitted will not be provided a response, but may be considered when formalizing the requirement.
