Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. CLIN 0001 Load and Transport the following items from U.S. ARMY HEALTH CLINIC, BLDG 5236 VALDEZ CIRCLE, DUGWAY, UT 84751 to U.S. ARMY HEALTH CLINIC, BLDG 5116 KISTER AVE, DUGWAY, UT 8 4751: 3 each Safes (1-4 drawer, 2-2 drawer); 4 each Rolling File Cabinets; 10 each Glass Medical Cabinets w/Supplies; 7 each Coat Racks; 5 each Refrigerators; 10 each Supply Shelves; 3 each Flammable Storage Cabinets; 4 each Typewriters; 400 each (Contents of Glass Med Cabinets, Office Contents, Exam Room Supplies, Lab Contents, Optometry Room (7-8 Boxes), Emergency Rooms (5-10 Boxes per room, no equipment), Supply Rooms, Medium size, 20-25 boxes per room, Medical Records Rooms, 140 total boxes, Pharmacy, s mall, 40 boxes); and 10 each Filing Cabinets. All padding materials and moving equipment shall be provided by the winning bidder. CLIN 0002 Pack, Load, and Transport the following items from U.S. ARMY HEALTH CLINIC, BLDG 5236 VALDEZ CIRCLE, DUGWAY, UT 84751 to DRMO HILL AFB, BLDG 896, HILL AIR FORCE BASE, UT 4056: 43 each Desks; 34 each Bookshelves; 30 each File Cabinets; 3 each Couches; 5 each Mobile Stretchers; 5 each Exam Tables; 24 each Tables, small and medium size; 4 each Conference room tables (large); 16 each Shelving, Supply type; 13 each Medical Tables, metal; 7 each Stools; 2 each A/V Carts; 81 each Chairs; 30 each Waiting Room chairs; 1 each X-ray Chart Viewer w/table; 19 each Computer Tables; 2 each Sterilizers (large); 1 each Ice Maker (Medium size); 1 each Blanket Warmer (medium size); 1 each X-ray machine w/ processor; 50 each Small, Miscellaneous Items; and 1 each Sound Proof Booth. All padding and packing materials and moving equipment shall be provided by the winning bidder. The proposed contract is set-aside 100% for Small Business concerns. For this acquisition, the Small Business Size Standard is $23.5M and the NAICS Code is 484121.
