Description

The Southern Arizona VA Health Care System, 3601 S. 6th Avenue, Tucson, AZ 85723, intends to establish a community opiate substitute treatment program for eligible beneficiaries. The contractor shall provide patient medical assessment, patient daily dosage, provide office space, administration support, supplies, telephone services, cooperative record keeping/support, and a group room for a VA Addiction Therapist to conduct required group sessions with VA patients. The services and space shall be provided by the contractor in the Tucson, AZ metropolitan area. The contractor shall have the following current licensure and accreditation: DEA License for Opiate Substitution, Arizona State License for Methadone Treatment, Accreditation from either JCAHO or CARF Accreditation from SAMHSA/Center of Substance Abuse. The contract shall be for a base period with four (4) option renewal years. This procurement is a 100% total small business set-aside (Note # 1). The applicable NAICS code is 621420. NOTE: THIS NOTICE MAY HAVE POSTED ON FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (30-JUN-2006).
