- 1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information in this notice. Solicitations will be made available on or around January 3, 2007. All requests for a solicitation must be in writing (i.e. letter, email, or fax). 2) Scope of Work: Diabetes Case Manager/GPRA Services. Contract period: February 26, 2007 through February 25, 2008. Project Goal: Improve the health status of Native American people with diabetes and Native Americans that are at risk for developing diabetes and cardio vascular disease. Activities to achieve goals: Clinic visit (1) within contract period at 23 California Indian Health Clinics (clinics using RPMS and/or accepting assistance with case management); on site clinic visit access: (a) Clinic organization (b) Diabetes/GPRA program structure (e.g. staff involved, education program, staff who enter diabetes-related data) (c) Use of Diabetes/GPRA Management System, DM Register, RPMS, CRS and QMAN reports to assist in updating register, evaluate patient status categories, etc. (d) Compare last year?s IHS diabetes audit/GPRA measures and run current reports on active clients (e) Case management system, if any; devise system and assist in implementation as needed (f) Review education needs (diabetes knowledge, RPMS, DMS, and EpiInfo) Attend (1) national diabetes data meeting with the Division of Diabetes; (1) California Area IHS and/or Tribal Advisory/Leader?s meeting Assist the California Area Office Area Diabetes Coordinator/GPRA Coordinators in other activities as need 3) Knowledge and skills required: (a) Crurrent CDE credential (b) Healthcare professional status (Registered Dietician) (c) Experience working with the RPMS/DM management and CRS databases (d) Substantial experience in diabetes care and CVD/chronic disease management (e) Knowledge of CRS and GPRA measures (f) Knowledge of Indian Health Service, tribal and urban Indian health care programs and health needs of Native Americans (g) Excellent communication skills, both verbal and written 4) The 23 California Area Indian Health Clinics/DM Case Management and 12 GPRA sites are: Chapa De Indian Health located in Auburn, CA Colusa Tribal Health located in Colusa, CA Consolidated Tribal Health located in Ukiah, CA Feather River Indian Health located in Oroville, CA Greenville Rancheria located in Greenville, CA Indian Health Council, Inc. Lake County Tribal Health located in Lakeport, CA MACT Health Board located in Tuolumne, CA Northern Valley Indian located in Willows, CA Oakland/San Francisco Urban Program located in Oakland, CA Redding Rancheria located in Redding, CA Riverside San Bernardino County located in Banning, CA San Diego American Indian Health located in San Diego, CA Santa Barbara Urban located in Santa Barbara, CA Santa Ynez Tribal Health located in Santa Ynez, CA Sonoma Indian Health located in Santa Rosa, CA Southern Indian Health, Inc. located in Alpine, CA Sycuan Medical/Dental located in El Cajon, CA Table Mountain located in Friant, CA Toiyabe Indian Health located in Bishop, CA Tule River located in Porterville, CA United American Indian Involvement, Inc.
