- This is a Market Survey to identify potential Service-Disabled Veteran-owned Small Business (SDVOSB) or Small Veteran-owned Business sources that possess the expertise, capability, and experience to provide Durable Medical Equipment (DME) services for the following Department of Veteran Affairs' facilities: VAMC, San Francisco, CA; VAMC, Palo Alto, CA; VAMC, Livermore, CA; VAMC, Martinez, CA; VAMC, Mather, CA; VAMC, Reno, NV; and VA Medical & Regional Office, Honolulu, HI. The Contractor shall furnish, deliver, set-up, recover, clean, repair, store, and reissue, new and used medical equipment to U.S. Department of Veteran Affairs (VA) beneficiaries' home or to the VA, as ordered by VA, in accordance with the terms and conditions of this contract. Contractor is responsible to establish warehouse space at the various locations to house VA-owned DME equipment. The North American Industry Classification System (NAICS) code is 811219 with a Small Business Size Standard of $6.5M. All interested parties must be registered in the Central Contractor Register (CCR) in order to receive an award. All eligible firms responding to this Sources Sought must ensure their response is complete and sufficiently detailed to allow the government to determine the firms' qualifications to perform this requirement. The Contractor must have experience in providing the same or similar services. The minimum level of acceptable experience required is 24 months within the previous 36 months. A Quality Control Plan will be required with the submission of the offerors proposal. Interested firms must provide the following information: Company Name, Address, Point of Contact, Telephone and Fax Number, Email Address and Business Size. Past Performance: List your three most recent contract awards for like or similar work, to include Client's Name, Point of Contact, Phone Number, Description of Services, Dollar Amount, and Period of Performance. Please provide your response by December 28, 2006. The Government does not intend to award a contract based upon this Sources Sought announcement. The Market Research does not constitute an invitation for bid or request for proposal and is not to be construed as a commitment by the Government.
