Description
?This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-15. This solicitation is a 100% set-aside for Small Business. The NAICS code is 621511, with a size standard of $12.5 million. The delivery, acceptance, and FOB point is the VA Sierra Nevada HCS in Reno, NV. The contract will include a one year basic period and two one-year options. Only electronic offers will be accepted. Offers are due to joel.burgess@va.gov by NLT 1:30 p.m. on December 21, 2006. The Description of Work and Price Schedule: Scope of Work: The contractor shall provide Liver Biopsies as described below. All procedures (excluding pre-biopsy) shall be performed at the contractor's facility. For pre-biopsy Jane Whipple, RN, CNS will be the VASNHCS contact for information. She can be reached at 328-1831. A history and physical from the referring doctor or facility will be provided. Current lab work with PTT, HCT, and HGB will be provided. No blood thinners (coumedin, aspirin or heparin) will be used for five days prior to the procedure. On the day of exam the patient will be required to arrive 30 minutes prior to the exam. NPO will be 4 hours prior to exam. The patient will stay two to four hours post exam. During post procedure the patient will be monitored by an RN, radiologist or other support staff for two to four hours following the exam. The patient will be resting on a gurney with BP, pulse and respiration monitored. The patient will be discharged with Radiologists approval. Once the patient is relatively stable after a complication following the procedure, the contractor will call the VASNHCS triage at 328-1214 and discuss appropriate transfer of the patient back to the VA. If the patient is unstable post procedure the contractor retains the option of calling the nearest emergency room and arrange for appropriate transfer. The contractor will notify the VASNHCS immediately. The contractor shall fax the biopsy report to VASNHCS to 328-1769 within 24 hours of the procedure. The VASNHCS will arrange for specimen pick up and transportation as necessary at no costs to the contractor for a specimen-handling fee. The cost of the biopsy shall include biopsy of liver, needle percutaneous, CT guidance needle biopsy, conscience sedation for patient comfort as well as seven (7) units of injectible sedatives.
