Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV07T0034. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect t hrough Federal Acquisition Circular 2005-05. The North American Industry Classification System Code (NAICS) for this procurement is 339111. This requirement is 100% total Small Business Set a-side. The Eisenhower Army Medical Center, Fort Gordon, GA, h as a requirement for the following items: CLIN 0001 - MAG 3 10 millicuries (mCi) per dose activity on the date of delivery as required. Radioactive material used in the performance for all diagnostic imaging. WILL CALL FOR DELIVERY AS NEEDED. THE PER FORMANCE PERIOD FOR THIS REQUIREMENT WILL BE 1 JANUARY 2007 THROUGH 31 DECEMBER 2007. QUANTITY: 55 EACH TOTAL PRICE FOR CLIN 0001 $___________. CLIN 0002  131 Iodine 30mCI dose per capsule calibrated for date requested. WILL CALL FOR DELIVERY AS NE EDED. THE PERFORMANCE PERIOD FOR THIS REQUIREMENT WILL BE 1 JANUARY 2007 THROUGH 31 DECEMBER 2007. QUANTITY: 80 EACH TOTAL PRICE FOR CLIN 0002 $____________. CLIN 0003  131 Iodine 200mCi dose per capsule calibrated for date as requested. WILL CALL FOR DELIVERY AS NEEDED. THE PERFORMANCE PERIOD FOR THIS REQUIREMENT WILL BE 1 JANUARY 2007 THROUGH 31 DECEMBER 2007. QUANTITY: 10 EACH TOTAL PRICE FOR CLIN 0003 $_____________. The Government anticipates awarding a single award for this requirement. Items will be delivered to Eisenhower Army Medical Center, Property Book Officer, Bldg 300, East Hospital Road, Fort Gordon, GA 30905-5650.
