- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for proposal (RFP-CMS-2007-0004). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-14. This procurement is set aside for Service-Disabled Veteran-Owned Small Businesses under NAICS code 423430, size standard 100 employees. Product listing /description of requirements: see attached Word document. The base period of performance is January 1, 2007 through December 31, 2007, with options through December 31, 2009. Delivery shall be made to CMS, Baltimore, MD. FOB Destination. The specific evaluation criteria are price and past performance. Past performance is approximately equal to price.
