Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E070010, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is a Small Business Set Aside. All small businesses shall provide a breakdown of their costs to demonstrate that at least 51% of the cost of contract performance incurred for travel, supplies, training materials, etc. shall be performed and incurred by the small business (prime). All small businesses shall comply with FAR 52.229-14 Limitations on Subcontracting. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 001: Basic Year. Provide all supplies, materials, and personnel to provide Occupational Therapy Services (OTR/L) to the Turtle Mountain Middle School, Belcourt, ND 58316 in accordance with the scope of work. The quantity is 880 and the unit is Hours. Please provide an estimate for your travel costs which shall be reimbursed at the actual travel costs in accordance with the Federal Travel Regulations. 002: Basic Year. Provide all supplies, materials, and personnel to provide Occupational Therapy Services (COTA/L) to the Turtle Mountain Middle School, Belcourt, ND 58316 in accordance with the scope of work. The quantity is 1592 and the unit is Hours. Please provide an estimate for your travel costs which shall be reimbursed at the actual travel costs in accordance with the Federal Travel Regulations. 010: OY4. Provide all supplies, materials, and personnel to provide Occupational Therapy Services (COTA/L) to the Turtle Mountain Middle School, Belcourt, ND 58316 in accordance with the scope of work. The quantity is 1592 and the unit is Hours. Please provide an estimate for your travel costs which shall be reimbursed at the actual travel costs in accordance with the Federal Travel Regulations. The contract shall be awarded as a firm fixed price contract with a basic and four one year option year periods. The Period of Performance (POP) for the basic year shall be from January 1, 2007 to June 30, 2007. The option year POP shall be from July 1 to June 30 for each subsequent year. All work shall be performed in accordance with the Scope of Work. Please provide a daily rate and estimated travel costs cost for all five years. Scope of Work. Provide on site services of an Occupational Therapist Registered/Licensed (OTR/L) and a certified Occupational Therapy Assistant/Licensed (COTAL) licensed in the state of North Dakota to secure professional occupational therapy for special education children and adolescents as needed five days per week during the regular school term and including the extended school year service time. Background: Services will be required five days per week during the regular school term and will include the extended school service time from June 4, 2007 through July 13, 2007. Contractor Tasks: 1. Contractor shall perform services for Turtle Mountain Special Education (TMSE) that will conform to all State and federal regulations including P.L. 93-112, PMC 101-476, 25 CFR part 454, 324-CFR, part 300. 2. On or before the tenth of each month, the contractor shall submit a statement to the TMSE indicating the charges for services rendered for the previous month. 3. Contractor agrees to perform its said work in strict accordance with currently approved methods and practices in its professional specialties. The contractor agrees that all Occupational Therapy (OT) services rendered will be in accordance with ethical and professional standards. The outcome is to ensure occupational therapy services shall be performed and rendered in a competent, efficient and satisfactory manner. 4. The relationship between TMSE and the Contractor is that of independent contractors. The contractor and the staff who will provide services, shall not be considered as employees, agents or representatives of the TMSE for any purpose whatsoever. 5. The contractor shall be responsible for all benefits, FICA, applicable taxes, worker?s compensation and unemployment insurance for its employees. TMSE will incur no further liability beyond the hourly rate paid to the Contractor for the occupational therapy services. 6. In the event, the Contractor incurs a staff reduction caused by an employee resignation at least a two week notice shall be given to TMSE. Every effort will be made to fill the position and re-instate services as quickly as possible. TMSE shall provide: 1. Space for OTR/L and COTA?L to provide OT services. 2. An on-site liaison to coordinate services for OTR/L or COTA/L. 3. Student files and information. Confidentiality ? all materials furnished or produced by the Contractor, relating to work performed, researched or derived under this contract, including but not limited to documents, reports and correspondence, will remain confidential. The contractor shall not release information to any persons, groups of persons, or organization other than the BIA, TMSE. Breach of confidentially may be grounds for rescission of the contract. 4. Adaptive devices or equipment. The services will be provided to approximately 42 students depending on need. Three are 42 students receiving direct services thirty minutes, two times per week, 17-20 students are seen 30 minutes daily. There are 9 students whoa re in evaluation status with ongoing referrals. The work will be conducted during a 24 hour work week, which may be increased or decreased. The contractor shall have a favorable background determination issued by the BIE, Security Office. QUALIFICATIONS OF THE EXPERT PROVIDER: 1. Experience in providing Occupational Therapy service to Bureau operated schools. 2. Degree in the field of Occupational Therapy with Certification to practice in the state of ND. A copy of current State certification of all employees performing services under this contract shall be provided. Please include your TAX ID Number, CAGE Number (obtain at www.ccr.gov) and DUNS Number when submitting your quote. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service and provide a copy of their credentials, including but not limited the college diploma. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the BIE, Office of Personnel Security and Suitability Program in accordance with the Background Investigations Clause. The Offeror shall provide the lump sum cost with a breakdown of their cost to provide this service and respond to the Evaluation Factors delineated at the end of this synopsis.
