Description
VA-261-07-RQ-0012 Offers Due: Jan. 12, 2007 Request for Proposal (RFQ) - Stent Consignment Agreement for Prosthetics Dept., VA Palo Alto Health Care System. The NAICS code is 423450; size standard of 100 employees applies. Product Service code of 6515 applies. The actual solicitation should be posted on or about January 3, 2007. Award will be firm fixed-price contract, FOB destination with payment in arrears on certified invoice. This is expected to be a sole-source consignment type procurement. The intended vendor is the only firm that provides the required items in the variety and exact specifications required by the VA. It has also been determined that it is not economically feasible to purchase and stock a variety of stents that may never be required. Furthermore, the intended consignment agreement will not monetarily obligate the VA for any stents until they are used. This is a five year requirement with one base year and four option years. Contractor is responsible for submitting all documents required in accordance with the solicitation, including any amendments which will be posted on this website. Solicitation is also subject to availability of funding. Quotations shall be submitted on an all or nothing basis. Partial quotations will be rejected. Quotations shall be valid for 60 days from offer due date.
