The following Statement of Work is in addition to the Combined Synopsis/Solicitation that was posted on 19 Sept 2006. Definitions 6 STATEMENT OF WORK LEASE OF HEAVY CONSTRUCTION EQUIPMENT 1. Scope Of Work: The contractor will provide all labor, parts, tools, material, delivery and pick-up, facilities, personnel, equipment, supervision, and any other items and/or services necessary, except that specifically identified as Government Furnished in para 6, to provide a D7 Dozer at Hemlock Bay, Annette Island, Alaska, as specified in this Statement of Work (SOW). Mission. The primary mission supported under this contract is the lease of a D7 Dozer with semi-u blade and multi-shank ripper in support of Operation Alaskan Road. Operation Alaskan Road is a Department of Defense (DOD) Innovative Readiness Training (IRT) Exercise. This USCINPAC sponsored multi-year exercise is an engineering effort, which consists of the construction of approximately 14.5 miles of mountainous and coastal roadway on Annette Island in Southeastern Alaska. Annette Island is located approximately 20 miles north of the US/Canada border. It is located 918 air miles from Anchorage and 687 air miles from Seattle. Air and Water are the only the lines of transportation with Ketchikan, Alaska which is the only major rural hub of the area. Therefore transportation of equipment will be via barge. Period of performance is 02 January 2007 thru 01 Jan 2008. 1.2 The contractor shall provide only equipment in good serviceable condition performing all its intended functions with no missing or broken parts 1.3 The contractor shall, within ten (10) working days after receipt of the written delivery order, coordinate with the Contracting Officer a date for inspection and acceptance of equipment. Inspection of the equipment will be at no additional cost to the government. 1.4 Upon completion of the inspection, the Contracting Office Technical Representative (COTR) must notify the Contracting Officer, in writing, of any equipment not acceptable. The COTR must state in detail the reason for the rejection and list all repairs needed to make the equipment acceptable. 1.5 Items added to the contract after the initial lease period will be subject to inspection and acceptance by the COTR under the same terms. 1.6 The COTR or designated representative will approve all parts up to $2,500 prior to purchase and installed on equipment. If repair parts exceed $2,500 approval is required by the Contracting Officer. All replacement parts required for maintaining equipment will be ordered within 24 hours from time of approval. 2. Contractor Training, Equipment Maintenance, Repair, and Travel: 2.1. Contractor Training: The contractor shall train the Joint Force Engineering Component Command (JFECC) operators and maintenance personnel on all functions, operations, daily maintenance, services and minor repair of leased equipment. Initial training shall be conducted onsite within the first five (5) days of equipment arrival, and shall be completed within five (5) workdays. Training will include the repair/servicing procedures, oversight, and quality assurance per type of equipment. 2.2. The Government will perform recurring servicing and minor maintenance on leased equipment including, but not limited to: cleaning, lubricating, adjusting, calibrating, part replacement, and minor repairs using industry standards and tools as outlined below. 2.2.1. Contractor Technical Representative: The contractor shall provide a Technical Representative within eight (8) hours of notification by the COTR to assist and advise the Government in the maintenance and repair of leased equipment. The Contractor?s Technical Representative shall take receipt of equipment identified by the COTR as beyond the repair capabilities of the Government. The Contractor?s Technical Representative will validate repairs and services performed by the Government to ensure they are in accordance with industry standards. 2.2.2. The contractor shall be responsible for performing all contractor repairs where the equipment is located. If repairs cannot be accomplished on site, the contractor shall be responsible for removal of the failed equipment and the delivery of the replacement equipment from/to Annette Island. 2.2.3. The maximum acceptable down time for contractor repairs of equipment identified as beyond the repair capability of the Government is five (5) working days. If contractor repairs are not completed within five (5) working days of notification by the COTR, the contractor shall provide acceptable equipment replacement or, prorate the monthly lease amount for each day not available for usage. For proration purposes, the daily rate shall be 1/30th of the monthly rate. 2.2.4. Completion of Repairs: Upon completion of contractor repairs, the contractor shall submit, in writing, an inspection report to the COTR within 24 hours of completion. The report must include the name of the person performing the repairs, the repairs made to the item, inspections completed, and date. 2.3. Equipment Maintenance and Repair: The contractor shall perform repairs (including parts and labor) of leased equipment as identified by the COTR when the repairs are deemed to be beyond the capability of the Government (the contractor shall be reimbursed for all labor, parts, and part(s) transportation at the contractor?s cost). . The COTR or designated representative will approve all parts up to $2,500 prior to purchase and installed on equipment. If repair parts exceed $2,500 approval is required by the Contracting Officer. Contractor acquired parts will be invoiced and reimbursed using CLINs 0x01AB, Parts/Materials, Contractor Acquired, at cost, not to exceed fair market value. 3. Equipment Wear: 3.1. Tire Wear: NA 3.2. Paint Wear: Upon delivery, leased equipment shall be jointly inspected and documented by the contractor and COTR for cosmetic appearance. Paint scratches that do not require body repair and that occur while being operated within design capabilities shall be considered fair wear and tear. Paint and body damages will be assessed at the end of the lease period will be handled IAW para 3.5. 3.3. Dozer Under-carriage Wear: All under-carriage wear (including lower rollers, idlers, upper rollers, track-links, and sprockets, etc.) that occurs during the lease period is considered fair wear and tear and part of the lease amount. THE CONTRACTOR IS RESPONSIBLE FOR THE UNDERCARRAIGE WEAR INCLUDED IN THE ABOVE AND BELOW DESCRIPTIONS. 4. Equipment Usage: The contractor shall allow 200 hours of usage per month not to exceed 2400 hours for the period of performance. Any use over 200 hours per month will be calculated at the hourly overtime rate. The overtime costs shall be billed on the existing equipment CLIN. 5. Equipment Early Turn-in or Lease Extension: Credit or charge to the Government for early turn-in or extension of equipment will be prorated based on the existing monthly leased amount. For proration purposes, the daily rate shall be 1/30th of the monthly rate. 6. Government Furnished Services: Following initial training, Government personnel shall conduct services, (IAW the manufacturer?s service and repair manuals) and repairs (including parts and labor) of leased equipment. Such as: ? The replacement of damaged hoses and belts needed to put equipment back into operation within twenty-four hours. ? Lubrication and oil changes ? Required emergency repairs preventing the discharge of fluids into the environment. ? Assisting in the timely completion of recurring/non-recurring services. Daily operator checks, adjustments and services as identified by the manufacturers Operations Manual. 6.1. The Government will provide maintenance documentation for all government repairs to the leased equipment and shall make documentation available to the contractor upon request. 7. Definitions: Contracting Officer (CO): A Government person duly appointed with the authority to enter into and administer contracts on behalf of the Government. The only person authorized to make changes to the contract. Contracting Officers Technical Representative (COTR): A Government employee who monitors and measures contractor performance on-site for technical issues pertaining to the repairs and associated costs. Working Days: Monday through Saturday.
