Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2005-15. This requirement is set-aside for small business only. The NAICS Code for this synopsis/solicitation is 541380, Size Standard $11 Million. Solicitation/Purchase Request number CEVCBPA1 is issued as a Request for Quote (RFQ). The Government intends to award a Five (5) year BLANKET PURCHASE AGREEMENT (BPA) to provide all parts, material, labor, equipment and supervision necessary for Environmental Sample and Analysis Services in accordance with attached Statement of Work. These services are for 377 MSG/CEVC, 2050 Wyoming Blvd SE, Bldg 20685, Kirtland Air Force Base, NM 87117 (FOB Destination). Reference Attachment # 1, Statement of Work, Dated 21 December 2006 Reference Attachment # 2, Department of Labor (DOL) Service Contract Act (SCA) Wage Determination (WD) Number 05-2361, Rev 01 Reference Attachment # 3, Past Performance Evaluation Questionnaire Award shall be made based on the best value to the government. Best Value shall be based on price, capabilities and past performance. All pricing (given for price list) shall be reflective of the various basic requirments for solids and liquids given in the Statement of Work (Attachment I). Include all rates for Base Rates and Labor Hours. In addition to providing a detailed Price List with their quotation, offerors must also include with their quotations the Past Performance Evaluation Questionnaire (Attachment # 3), from at least three (3) sources with and for whom they have previously performed this type or similar service within the past five (5) years. A Statement of Capabilities (SOC) of less than five (5) pages is also required to outline the offerors ability to perform on this BPA. Any needed amendments shall be posted on the affected solicitation page EPS site. It is the offeror?s responsibility to monitor this site for the release of amendments (if any).
