- The Department of Veterans Affairs is soliciting for health care providers capable of furnishing complete Community Based Outpatient Clinic (CBOC) primary care services for two (2) CBOCs in separate geographic areas for its' eligible beneficiaries as referred by the Carl T Hayden VA Medical Center (CTHVAMC), 650 E Indian School Rd, Phoenix, AZ 85012-1892, as the parent facility in accordance with all terms, conditions, provisions, schedules and specifications of this solicitation, under the direction of the Chief of Staff and the Ambulatory Care Clinical Services Administrator. This procurement is conducted under the authority of Public Law 104-262 and 38 U.S.C. 8153. One CBOC will serve veterans primarily residing in the geographic area of Payson, Arizona (Gila County) a rural city located approximately 90 miles northeast of Phoenix, Arizona on State Route 87, and approximately one hour and 36 minutes from the parent CTHVAMC. There are approximately 571 unique veterans in the Payson area eligible for care. The second CBOC will serve veterans primarily residing in the geographic area of Buckeye, Arizona (Maricopa County) a city located approximately 36 miles west of Phoenix but within the metro-Phoenix area and 41 minutes from the parent CTHVAMC. There are approximately 446 unique veterans in the Buckeye area eligible for care. Services at both CBOCs will include Primary Care only, and all mental health care and specialty care will be referred to the CTHVAMC. Contract health care providers must complete credentialing and privileging in the VA VetPro system as part of the contract requirements, and this credentialing and privileging requirement is non-negotiable. Low risk background investigations shall be conducted on all contractor personnel, and the cost of these investigations shall be borne by the contractor. Evidence of prior successful background investigations can be submitted with the offer. The NAICS codes for this requirement is 621112, size standard $9.0M. The solicitation is unrestricted full and open competition. The anticipated contract period will be for a base period from April 1, 2007 through March 31, 2008, with four additional 12-month option periods through March 31, 2012. The solicitation for commercial services will be conducted in accordance with FAR Part 12, Commercial Items, and VAAR Part 873, Simplified Acquisition Procedures for Health-Care Resources. The solicitation will be available on or about January 8, 2007, with offers due on or about February 8, 2007. Hard copies of the solicitation will not be made available. Do not add dashes or any spaces to the solicitation number in the search engine. The solicitation is currently undergoing a legal and technical review by the VA Medical Sharing Office and is subject to amendment resulting from the review. Contractors are advised that they are responsible for periodically checking Fed Biz Opps website and obtaining any amendments to the solicitation.
