Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ). This is a 100% Small Business Set-Aside.The point of contact for this RFQ is Amn Daniel Blackmore at (660) 687-5425. STATEMENT OF WORK FOR MAINTENANCE OF OIL/WATER SEPARATORS (OWS), 1. DESCRIPTION OF SERVICES. The contractor shall provide all labor, tools, equipment, materials, supervision, transportation, and all other necessary items and services necessary to maintain oil/water separators located at Whiteman AFB, MO. Work shall comply with government and commercial standards. See attached map for location, size and frequency of cleaning of oil/water separators. SPECIFICATIONS AND DESCRIPTION OF WORK. 1.1.1. FULL SERVICING. Once a year, in April, the following services shall be performed: 1.1.2. All filters and screens shall be removed and cleaned, then reinstalled. All sludge and debris removed from filters shall be properly disposed of in accordance with federal, state, and local regulations. 1.1.3. The contractor shall pump all oil, oil residue, and other waste from separators and catch tanks. Drain separator and catch tank completely. Inspect all parts for problems. Wash and drain the separator and fill with clear water as required for the unique configuration of the separator. The contracting officer?s authorized representative will locate the water source(s). Transport all materials from oil/water separators to a licensed disposal facility in accordance with federal, state, and local regulations. 1.1.4. PARTIAL SERVICING. Partial servicing will be performed in October during the option years. 1.1.5. During pumping, all oil and only a minimum amount of water shall be removed. 1.1.6. SEMI-ANNUAL SAMPLING REQUIREMENTS. Prior to servicing each separator, the government will collect one discrete sample from each separator for testing. The government will provide test results upon request. 1.1.7. The government will have the sample analyzed for the parameters listed in 40 CFR Part 279.11. If the sample test result exceeds any parameter, the government will notify the contractor prior to pumping. 1.1.8. The contractor shall comply with the following list of federal regulations pertaining to liquid wastes: Resource Conservation and Recovery Act (as amended by the Hazardous and Solid Waste amendments of 1984), subtitle C, ?Hazardous Waste Management.? Oil Pollution Act of 1990, Section 1001. The Clean Water Act, Title 3, ?Standards and Enforcement?, and Title 4, ?Permits and Licenses?. Title 40 CFR 260-282, ?Hazardous Waste? Title 40 CFR Parts 104-149 ?Water Programs?. Title 40 CFR Part 110, ?Discharge of Oil?. Title 40 CFR Part 112, ?Oil Pollution Prevention?. Title 40 CFR Part 401-471, ?Effluent Guidelines and Standards?. Title 40 CFR Subpart F, ?Releases from Solid Waste Management Units?. Title 29 CFR 1910.1200, Hazard Communication Title 29 CFR 1910.147, Lockout/Tag out Title 29 CFR 1910.146, Confined Space Title 29 CFR 1910.120, Hazardous Waste Operations and Emergency Response 1.1.9. EMERGENCY SERVICE CALLS. Emergency services may be required in the event of natural disasters or other unscheduled repairs of pipes or other infrastructures involving oil/water separators. In the event that servicing is required outside the contractor?s schedule, the contractor shall begin the service within 12 hours after notification. Notification may include telephone notice to the contractor. A voice message left on an answering machine or with an answering service shall constitute notification. Emergency service calls shall be priced according to the rates in the Unit Price List, subCLINs located on the bid schedule dtd 21 Sep 06. (Ref CLINS 0003, 1003, 2003, 3003, and 4003). 1.1.10. SAFETY AND HEALTH STANDARDS. The contractor shall initiate and maintain programs to comply with the provisions of the Occupational Safety and Health Standards Act concerning entry requirements in confined spaces and handling potential hazardous substances in accordance with OSHA standards set forth in Title 29 of the Code of Federal Regulations (OSHA regulations can be found in parts 1904, 1910 and 1926). 2. SERVICE DELIVERY SUMMARY. SDS OBJECTIVE SOW REF SDS THRESHOLD METHOD OF SURVEILLANCE Clean and Maintain Oil/Water Separators and Emergency Catch Tanks. Initial check documented. Document all safety hazards on all separators and catch tanks during inspections. Remove and properly dispose of all reclaimed material. Clean all systems and fill with clean water as required. Ensure all tanks are in good working order. 1.1.-1.4. 100% 100% Inspection Emergency Service Calls. Service calls handled promptly and all work completed as specified. 1.5. 100% 100% Inspection Quality Control Plan 4.1 100% Quarterly audit of Government and Contractor Records 3. GENERAL INFORMATION. 3.1.1 QUALITY CONTROL. The contractor shall develop a quality control plan (QCP) that clearly identifies procedures to ensure compliance with terms of the contract and prevention of defective services. The plan shall identify a quality control inspector and phone number whom the government can discuss quality issues. 3.1.2 GOVERNMENT REMEDIES. The government may exercise its rights under FAR 52.212-4, Contract Terms and Conditions for Commercial Items, for contractor?s failure to correct nonconforming services. 3.1.3 HOURS OF OPERATION. All work shall be performed between the hours of 7:30 a.m. to 4:30 p.m., Monday thru Friday, except for emergency services. Partial servicing shall be performed in October and full servicing will be performed during the month of April or as directed by the CO or his authorized representative. 3.1.4 WORK SCHEDULE. The contractor shall submit its planned work schedule to the CO or his authorized representative not later than 15 days prior to the scheduled service. This schedule must specify the date and day of the week that the services are to be performed, and approximate time pumping and/or cleaning individual separator(s) is planned. The schedule is subject to approval by the government inspector. Should circumstances prevent the contractor from performing in accordance with the approved schedule, the contractor shall immediately notify the QAE inspector and the contracting officer for rescheduling. The contractor is advised that events such as exercises or other activities may require the contractor to change its schedule. Although the government will make reasonable efforts to provide advance notice, in some circumstances, adequate notice may not occur. Such schedule changes shall not be grounds for a claim against the government. All quarterly and annual services shall be completed within one week from commencing work for the period in which the service is scheduled, unless weather conditions or other events prevent completion within the one week period. 3.1.5 SERVICE TICKETS. Service tickets will be submitted with the building number noted and the quantities pumped from each location. All tickets will be signed and certified by the designated contract representative. 3.1.6 MANIFESTS. The contractor shall prepare and provide to the government for review, all necessary manifests required to lawfully transport used oil from Whiteman AFB. The contractor shall meet requirements in the Missouri Hazardous Waste Regulations, relating to marketers of used oil. 3.1.7 SECURITY REQUIREMENTS. The contractor shall obtain the following pass and identification items: all necessary cards, passes, button, decals, or other items required for access to the areas in which work will be accomplished. The CO will furnish a letter to obtain these identifications. The contractor will be required to apply for individual and vehicle identifications at the Whiteman AFB Visitor Center. Upon completion or termination of this contract, or termination of an individual?s employment under this contract, the contractor will retrieve the identifications issued and surrender them to the 509th Security Force, Pass and ID Section. 3.1.8 PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER All services specified in Section 1 of this contract shall be required in the event of a declared crisis that is implemented by the Air Force. The government will notify the contractor if and when this occurs. 3.1.9 SPECIAL QUALIFICATIONS. Contractor personnel shall obtain and maintain all federal, state and local certification requirements in maintenance of oil/water separators. 4. APPENDICES. A. Estimated Workload Data B.
