Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotation s are being requested and a written solicitation will not be issued. Solicitation number W91YTV-07-T-0032 is issued as a Request for Quote (RFQ). This acquisition is issued as an Unrestricted Open Market Procurement to obtain the maximum amount of competi tion from all responsible business concerns. The North American Industry Classification System Code (NAICS) is 811219; the Standard Industrial Classification Code (SIC) is 7699. This requirement is for Annual Maintenance Services on various types of gove rnment owned medical equipment as listed: (1 EA) Philips X-Ray Tomography Unit, Model #171950-D and (2 EA) Philips Irix Nuclear Medicine Cameras inclusive of on-site remedial repairs, emergency repairs, routine preventive maintenance services, emergency ca lls, replacement parts, shall include all systems subcomponents and assemblies which were part of the original system purchased. All subsequent maintenance provisions shall apply hardware, firmware and software, labor, parts, material in accordance with m anufacturer specifications and the Performance Work Statement. Please quote pricing based on issuance of a 12 month contract award. CLIN 0001: Period of Performance: 15 Feb 2007 15 Feb 2008. Annual Maintenance of Philips X-Ray Tomography Unit (1 EA) an d Philips Irix Nuclear Medicine Cameras (2 EA) and any associated components located at Moncrief Army Community Hospital. Provide labor/parts/material/travel & equipment necessary to maintain in safe operating conditions the listed Government owned equipm ent in accordance with manufacturer specifications and the Performance Work Statement: $__________________________________________________________________; CLIN 0002: Contractor Manpower Report: The Contractor is required to comply initially and annually with the requirements of this clause as stated in the Performance Work Statement. This Line Item provides funding for the mandated Contractor Manpower Report and covers a one-time set-up fee. The requested preventive maintenance services are for government owned medical equipment manufactured by Philips. Award will be issued based on the best overall value for the government.
