- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number AG-5540-S-0001. is issued as a Request for Quotes. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-15. The clauses and provisions referenced in this solicitation can be found in full text format at http://www.arnet.gov/far. This solicitation is 100% Set Aside for Small Business. The contract type will be firm fixed price. The associated NAICS code is 561720 and the small business size is $ 15 million. The following Department of Labor Wage Determination applies to this procurement: 2005-2421. The full text of the wage determination is included as an attachment. The United States Department of Agriculture, Farm Service Agency (FSA) announces a Request for Quote (RFQ) for Janitorial Services in the: Auglaize County FSA Office, 110 Industrial Dr., Suite E, Wapakoneta, OH 45895. The leased property is 3,055 square feet with restroom space not included in that total square footage. Contiguous Counties, Hardin and Van Wert, will require janitorial services, as well, they are being solicited under separate cover: AG-5540-S-0002 and AG-5540-S-0003, respectively. The Government will contemplate the award of multiple contracts to the same contractor, if it is in the best interests of the Government. When submitting a response, indicate the counties in which you have an interest. The Government anticipates the award of one or multiple Firm Fixed Price type of contracts resulting from this combined synopsis/solicitation. Responses should include the experience/qualifications of the contractor, personnel, equipment and services to perform the type of tasks described by this notice. High St. Rm. 540, Columbus, OH 43215-2495 STATEMENT OF WORK SERVICES REQUIRED The service provider shall keep the premises clean. Daytime cleaning is encouraged but interference with FSA business shall be avoided. The service provider shall provide supplies and equipment. The following schedule describes the level of services required. Performance will be based on the County Executive Directors (CEDs) evaluation of results, not upon frequency or method of performance. The total area of the office is 3,100 square feet. Daily: Empty trash receptacles and clean ashtrays. Sweep entrances, lobbies, corridors, stairs, and spot sweep offices. Clean drinking fountains. Sweep and damp mop or scrub toilet rooms. Clean all toilet fixtures and replenish toilet supplies, such as toilet paper, paper towels, and soap. Dispose of all trash and garbage generated in or about the building. Spot vacuum carpets. Dust desk surfaces. Weekly: Completely sweep offices and vacuum carpets. Dust furniture. Buff resilient floors in corridors, entrances and lobbies. Scrub or wet mop hard floors; sweep garages, storage space, and outside paved areas. Police sidewalks, parking areas, garages, driveways, grounds and shrubbery. Monthly: Damp wipe toilet wastepaper receptacles. Buff resilient floors in office space. Spot clean all wall surfaces within 70 inches of the floor. Three Times Annually: Vacuum or dust all surfaces in the building over 70 inches from the floor. This includes window blinds and light fixtures. Vacuum all drapes in place. Strip, wax, and buff resilient floors. Use non-slip floor wax. Twice Annually: Wash all glass surfaces, window blinds, roller shades, and light fixtures. Once Annually: Shampoo carpets in heavy traffic areas, including corridors and lobbies. PERFORMANCE TIME Initial 12 month period commencing: May 1, 2007 ending: April 30, 2008 1st option year, period commencing: May 1, 2008 ending: April 30, 2009 2nd option year, period commencing: May 1, 2009 ending: April 30, 2010 3rd option year, period commencing: May 1, 2010 ending: April 30, 2011 4th option year, period commencing: May 1, 2011 ending: April 30, 2012 SUBMISSION REQUIREMENTS One copy of signed SF 1449 Form, 1 copy of FAR 52.212-3 Offer Representations and Certifications -- Commercial Items (Nov 2006), available at http://farsite.hill.af.mil/vffara.htm, must be received at the following address: State of Ohio FSA Office, 200 N.
