- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, FA2517-07-T-6029, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-02. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 611710 and the size standard is $6.5 million. The following commercial item is requested in this solicitation; STATEMENT OF WORK MIVER SELF-STUDY 8 Jan 2007 Services (Non-Personal) contract for the MIVER Self-Study Coordinator to write and submit the required self-study document for Peterson AFB, CO 80914. Period of Performance is 05 Feb 07 to 30 Apr 07 covering 468 hours of work. MIVER SELF-STUDY COORDINATOR STATEMENT OF WORK (SOW) 1. DESCRIPTION OF WORK. a. Conduct research, write and coordinate the MIVER Self-Study for the 21st Mission Support Squadron, Education Office. The following areas will be included in the final document in accordance to Air Force / DOD Guidance: Table of Contents Welcome Letter Introductory Summary Principle 1. Mission Statement and Command Support Principle 2. Program Management and Leadership Principle 3. Student Services Principle 4. Resources Principle 5. The Voluntary Education Program Plan Appropriate Attachments b. The contractor will be responsible for ensuring that the final product meets the deadlines and project requirements set forth by the MIVER Team Chair and as prescribed by the Education Services Officer (ESO). The ESO will have final approval authority of the submission and will be the Quality Control Official on the project. d. Contractor will be provided access to the base and the Government will provide all necessary equipment to conduct the work. Government provided equipment includes, but is not limited to, furnished office space, furnished classrooms, storage containers for storing course materials and tests, office supplies and equipment, paper, computer?s, fax machine, etc., e. Payment will be made through the Wide Area Work Flow system on a monthly basis. 2. GENERAL INFORMATION. a. Contractor is authorized to obtain and use related materials in accordance with local procedures or guidance from the MIVER team and Education Services Officer. b. Contractor will independently conduct necessary research and business during normal duty hours and perform work according to strict time management requirements to ensure completion of the project by 30 April 07. c. Contractor will maintain records to document timelines, milestones and report to Education Services Officer on progress of project during regular business hours on a weekly basis. d. Contractor will present and prepare pertinent questions/issues and submit to the Education Officer for response or forwarding to HQ AFSPC or MIVER Team Chair as appropriate. 3. CONTRACTOR QUALIFICATIONS. a. Contractor shall have a specific knowledge of the Peterson AFB Education Services Program and local colleges / universities supporting the base. b. Contractor shall have a working knowledge of the MIVER program and how to write the self study. c. Contractor shall have knowledge and experience with the various programs covered in the self study such as the Tuition Assistance Program, Base Testing Program, Education Counselor Programs and academic programs offered on the base. Qualifications: Contractor shall have U.S. Air Force specific knowledge of the MIVER program and have performed a MIVER study on an U.S. Air Force installation since 2005. Past performance will be evaluated on five (5) ratings: exceptional, very good, satisfactory, marginal and unsatisfactory. Services will be conducted at Peterson AFB, CO Base Education Office. The following provisions and clauses apply to this acquisition: The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items. FAR 52.212-2(a) Quote will be based on price and past performance, both are approximately equal in importance. The provision at FAR 52.212-3, Offeror Representations and Certification. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items. Specifically, the following clauses cited are applicable to this solicitation: FAR 52.222-26, Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; FAR 52.222-36, Affirmative Action for Handicapped Workers; FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era. The clause at FAR 52.224-1, Privacy Act Notification and FAR 52.224-2, Privacy Act; FAR 52.232-33, Payment by Electronic Funds-Central Contractor Registration. The clause at DFARS 252.204-7004, Required Central Contractor Registration; DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statues or Executive Orders; Ombudsman (AFFARS clause 5352.201-9101 as prescribed 5301.9103); and FAR 52.222-21, Service Contract Act of 1965. All firms must be registered in the Central Contractor Registration (CCR) database at www.ccr.gov, Online Representations and certifications (ORCA) at https://orca.bpn.gov/ and Wide Area Workflow (WAWF) at https://wawf.eb.mil/ and will be verified prior to consideration of award. DUNS, Cage Code, tax ID, terms and conditions shall be submitted with quote. Quote Submission: include price information and past performance. Submit less than a one-page statement explaining the past performance and how it is recent and relevant of the U.S. Air Force specific MIVER self-study.
