Description
SCOPE OF WORK a. The contractor shall provide, when requested by VA, the services of qualified personnel to provide services stated herein at VA Connecticut Healthcare System (VA), 950 Campbell Avenue, West Haven, CT 06516 and/or at 555 Willard Avenue, Newington, CT 06111. When services are requested by VA, the contractor shall provide VA with resumes of all individuals capable of providing the services. VA shall reserve the right to interview prospective candidates prior to final selection. b. In accordance with Health Insurance Portability and Accountability Act of 1996 the contractor will be required to sign a Business Associate Agreement (Attachment 1). c. Contractor's file clerks(s) duties will include but not be limited to the following: 1. Qualifications/Basic Requirements - Must have knowledge of general office automation systems, including MS word, and must be skilled in verbal and written communication. 2. Files medical documentation into medical and administrative folders. 3. Creates new medical and administrative folders. 4. Retrieves folders from pull lists. 5. Reviews filed material for accuracy and repairs worn folders. 6. Copies medical documents for billing. 7. Delivers charts to inpatient wards and clinics. 8. Enters and retrieves data from mainframe network. 9. Shall be able to identify and correctly file an assortment of medical documents. DEFINITION OF WORK HOURS Hourly rate - Services shall be provided during the hours of 8:00am through 4:30pm, Monday through Friday, excluding federal holidays. This includes a one-half hour lunch break, which is at no cost to VA. Contractor's employee(s) will be required to be on site at the VA in West Haven or Newington during these times and must be performing the required services specified in the contract or the contract cost will be decreased accordingly during each billing cycle. Contractor shall be paid only for actual worked performed. Actual performance period shall not commence until contractor's employee(s) are brought on under this agreement. Contractor's employee(s) will be brought on for a minimum of 13 weeks (as long as performance is acceptable). After the initial 13 week period, VA reserves the right to continue/discontinue services with the current contractor's employee(s). If contractor's employee(s) performance is deemed unacceptable by VA, VA will notify contractor. The contractor will be required to contact their employee and the employee's services will be terminated immediately. It will be VA's sole determination as to whether an employee's services are deemed acceptable or not.
